AFFIDAVIT OF CARE AND CUSTODY

I, MA. ERNA O. ESTACIO, Filipino, of legal age, and a resident of No.

298 Marulas A Street, Barangay 36, Caloocan City, after having been
duly sworn to in accordance with law do hereby depose and state:

1. That I am the grandmother and/or legal guardian exercising

parental authority to the minor, Gerard Kean E. Dimalanta;

2. That the child is entrusted to me by her mother, my daughter,

Girlie E. Dimalanta since he was eight (8) years old;

3. That the child is under my care and custody since her mother
left to work in France;

4. That the mentioned child is petitioned to migrate in France by

her mother who is working at the said country;

5. That this affidavit was executed for the purpose of attesting to

the truth of the foregoing facts and for whatever legal purpose it
may serve;

AFFIANT FURTHER SAYETH NAUGHT.
