Introduction

5nos intermediate product storage tanks was offered for Format-1. Tank wise
inspection punch points of Format-I are given in this report.

1. 52-TT-CR-05D (DHDT feed)

External

 Voids were observed between annular extensions and foundation pad throughout tank
periphery.
 Foundation pad is damaged at annular extension plate. Same shall be restored by
bitumen premix. Foundation Slope shall be maintained toward outer.

Internal

 Bottom plate paint was found missing at drain line support location.
 Paint DFT of drain pipe bottom portion was found 100 microns in place of 300 microns.
 Manhole cover of shell and roof is not painted from internal side.
 Paint was applied without surface preparation on bottom support of Steel well pipe.

2. 52-TT-CR-6B (NHT feed)

External

 Voids were observed between annular extensions and foundation pad throughout tank
periphery.
 All anchor bolts nuts were found loose.
 Gap was found around anchor bolts at certain locations. Same shall be filled properly by
bitumen premix.
 Foundation pad slope was found toward annular extension ring. Slop shall be maintained
toward outer.

Internal

 Paint DFT of drain pipe bottom portion was found 70 microns in place of 300 microns.
 Manhole cover of shell and roof is not painted from internal side.

DFT less than required at bottom Voids found b/w annular ring and
portion of drain pipe foundation pad.

3. 52-TT-DR-18B (MTBE)

External

 Voids were observed between annular extensions and foundation pad at certain locations.
 All anchor bolts nuts were found loose.
 Gap was found around anchor bolts in foundation at certain locations. Same shall be filled
properly by bitumen premix.

Internal

 Drain line clamp supports installation balance.

 Manhole cover of shell and roof is not painted from internal side.
 Paint DFT of drain pipe bottom portion was found 100 microns in place of 300 microns.
 All nozzles & manhole cover of roof is not painted from internal side.
 Paint blisters is observed at weld joint between 1st & 2nd shell course plates and on 2nd
shell course plate at certain locations.

4. 52-TT-DR-08B (HGU Feed)

External

 Voids were observed between annular extensions and foundation pad at certain locations.
 All anchor bolts nut was found loose.
 Gap was found around anchor bolts in foundation at certain locations. Same shall be filled
properly by bitumen premix.

Internal

 Drain line clamp supports installation balance.

 Paint DFT of drain pipe bottom portion was found 100 microns in place of 300 microns.
 Manhole cover and product inlet nozzle of shell is not painted from internal side.
 Manhole cover and all other nozzles of roof is not painted from internal side.

5. 52-TT-CR-02D (VGO)

External

 Voids were observed between annular extensions and foundation pad at certain locations.

Internal

 Steam coils clamp supports installation balance at certain locations.

 Paint DFT of 2nd shell course plate was found less than 100 microns in place of 300
microns.
 Paint blisters was observed on 2 nd shell course plate at certain locations.
 Manhole cover of shell & roof and COD is not painted from internal side.

As per OISD-132 all safety valves shall be tested at BORL-work shop before installation at
