virtue of these presents, do hereby sell, convey and transfer unto said VENDEE,
JOSEPH DOMINIC N. HERMOSORA, his heirs, assign and successors in interest, the
above describe vehicle.

That I further declare that the said described vehicle is free from liens
and/or encumbrances and shall defend its ownership from any /all
claims/whatsoever.

and a resident of Cuanos, Minglanilla, Cebu, Philippines, after
having been duly s worn to in accordance wi th law, hereby depose
and say:

That I am a buyer of a certain Motorcycle still registered in the

name of DOMINADOR B. HERMOSORA , under CR No. 2951563-6, with
Chassis No. 4PF-158387 Engine No. 4PF-158387, and Plate No. 7T4497;

That on March 13 , 2019, at around 5:30 A.M., more or l ess, I

misplaced the Certificate of Registration of the said vehicle and
could not anymore be found notwithstanding my diligent efforts to
locate the same, henc e considered as lost for all legal intents and
purposes;

That said Certificate of Registration was not confiscated by

any government authorities in violation of R. A. 4136 and other
related laws;

That this affidavit i s executed to attest to the foregoing facts

for whatever legal purpose it may serve;

That the contents of this Affidavit are true and correct

according to my best knowledge and belief and that the same is
executed without any fraudulent intention against any person or
entity, natural or jur idical;
