CHECKLIST OF REQUIREMENTS (For Pag-IBIG Housing Loan Restructuring and Penalty Condonation Program) BASIC REQUIREMENTS (UPON LOAN APPLICATION)

2. Original copy of updated Real Estate Tax Receipt or Tax Clearance and Tax Declaration 3. Proof of Income (see details at the back) REQUIRED DOCUMENTS (UPON LOAN APPROVAL)

2. Real Estate Mortgage 3. Duly accomplished/notarized Promissory Note 4. Deed of Revocation of Notice of Cancellation (For CTS Accounts with Notice of Cancellation) For other applicable requirements, please refer at the back portion of the form. Pag-IBIG Fund reserves the right to request additional documents to facilitate loan restructuring evaluation process. (V02, 07/2016) INCOME DOCUMENTS: ADDITIONAL REQUIREMENTS (if applicable only) For Locally Employed, any of the following: Upon Loan Application a. Notarized Certificate of Employment and Compensation (CEC), Deed of Extra-Judicial Settlement of Estate indicating the gross monthly income and monthly allowances or For OFW members monthly monetary benefits received by the employee b. Latest Income Tax Return (ITR) for the year immediately Special Power of Attorney (SPA) notarized prior to date of departure. For preceding the date of loan application, with attached BIR Form OFW member abroad, a SPA notarized by a Philippine Consular Officer, No. 2316, stamped received by the BIR or SPA notarized by a local notary (of the country where the member is c.
