I hereby provide my consent to the QMaths Ramanujan Maths Club to issue Membership to my child/ward,

studying in Class__________ for the programme applicable to his/her class/section starting from the date
20th April 2019.
This programme has been exclusively designed and is being executed by the QMaths team from QMaths
Center of Learning at your child's/wards' School premises. Please submit same to your child's/wards'
respective class teacher by ________________________.

During the first day of the attendance of this programme, the parent/guardian need to accompany the
child to fill in the registration format.

The programme will be held from 4:00pm to 5:30pm twice a week. The day schedule will be shared by the
respective QMaths faculty on the day of registration.
