ACKNOWLEDGEMENT, WAIVER AND RELEASE FROM LIABILITY

I acknowledge that the potential for injury exists in volunteer assignment(s) and hereby

assume the risks of volunteering. In consideration for allowing me to participate in

volunteer assignments, I hereby (a) waive, release and discharge from any and all

claims or liabilities for death, personal injury, property damage or damages of any kind

that arise out of or relate to my participation in volunteer assignments, the following

persons or entities: Manila City Hall, Manila Zoological Botanical Garden, Zoo Crew

Philippines, and their respective officers, agents, servants and employees, and (b)

agree not to sue any of the persons or entities mentioned above for any of the claims or

liabilities I have waived, released and discharged. Should I become a volunteer, I agree

to comply with all the rules and regulations of the Manila Zoological and Botanical

Garden and Zoo Crew Philippines.
