AFFIDAVIT OF UNDERTAKING

I, Wendy So, of legal age, Chinese, with residence and postal address at Supreme
Garden, Padre Algue St. Tondo Manila, after having been duly sworn to in accordance with law
hereby depose and state that:

1. I am the companion of minor Gary Lester S. Tan who together with me will travel to
HongKong from May 2, 2018 to May 8, 2018 for Vacation/tour;

2. That said minor is the son of Gerald Dizon Tan and Lutgarda Sy Tan who gave their
full consent and permission to me as companion of their minor son;

3. I am a close family friend of Gerald Dizon Tan and Lutgarda Sy Tan;

4. I hereby undertake and affirm that I, together with the minor child will return to the
Philippines as soon as we finish the duration of our vacation/tour in HongKong and
promise to present him/her to the Travel Clearance Office, DSWD-NCR 3days upon
arrival;

5. I assume full responsibility over the minor’s safety and welfare during the entire
duration of our travel and stay at Flat C 28/F Block 3 Elegance Garden, Tuen Mun
HongKong and hold the staff, officers and or any employee of DSWD-NCR free and
harmless from all and any liability arising from travel of the said minor;

6. I executed this affidavit to attest to the truth of the foregoing facts and for whatever
legal purpose this may serve.
