This has reference to the discussion with us on the subject. In terms of the discussions we have agreed to
accept your services for clearance of the wet and dry garbage

Amount as follows: You shall deposit a sum of Rs. ……………………………………per month in advance at
the beginning of every month. Copy of payment receipt issued by accounts department should be submitted
at security control room from before or in the beginning of every month.

SCOPE OF WORK

 You will ensure to clear the wet & dry garbage from the hospital every day to ensure that garbage is
cleared by the specify time your staff and your vehicle must reach the hospital an hour before the
same is removed from the hospital premises.

 The item comprising dry garbage would be papers, card board, empty bottles, empty tins etc. You will Commented [WU1]: Empty Bottles
ensure that the requisite form for clearance is duly signed before the same is allowed to move out of
the premises.

 You will ensure to deploy such number of staff and also ensure the availability of rickshaw / tempo /
tractor trolley to ensure compliance of the schedule time for removal.

 It shall be entirely your responsibility to ensure that the wet / dry garbage removed is disposed off at a
place specified by the local authorities / Municipal Corporation / State Government. The Hospital shall
not be responsible for any violation in regard to dumping of the same by your staff while disposing the
said garbage.

 You will be responsible for cleaning the wet garbage and dry garbage area after clearance of the
same including the loading bay. In case the clearance is not done within the stipulated time every
day, hospital management has the right to depute any other agency at your cost.

  The garbage so removed and transport deployed for removal shall confirm to such rules/regulations
as may be prescribed by local authorities / Municipal Corporation / Police Department / Transport
Authority covering valid permit specific licenses etc.

 You shall ensure that the garbage so taken for transportation is duly covered and is not openly
exposed or fall on the road while being transported to the dumping site.

 You shall be solely responsible to comply with the local law for deployment of staff, using of proper
transport and also such other rules regulations as may be imposed during the tenure of this contract
either by local authorities or by the State Government 0of Central Government, as the case may be, Commented [WU2]: or
in execution of this contract.

 Should any such dispute arise during the tenancy of this contract, the decision of the management of
the hospital shall be final and binding upon you. You will not be heard nor will you claim any
compensation in regard to any violation made in the course of execution of the contract.

 You will be required to submit the details of the staff who shall accompany the rickshaw / tempo /
tractor trolley entering the hospital premises for clearance as valid entry passes will be issued during
the tenancy of this contract. Should be persons accompanying the rickshaw / tempo / tractor trolley
not carry the entry pass they will be denied entry in the hospital premises. In such an event the
hospital reserves the right to ensure the clearance of the area for cleanliness and also dispose of dry
garbage to whomsoever it may deem fit. You will not claim any compensation for the same.

 The management of hospital reserves the right to cancel the contract after giving 30 days notice to
you.

This word order is being send in duplicate please return one copy of the same, duly signed by authorized
signatory of the company, to us as token of the same above terms and conditions.
