AFFIDAVIT OF MARITAL CONSENT

I, _________________________, Filipino, of legal age, married to

sworn to in accordance with law, hereby depose and state that:

1. My spouse and I have agreed and have decided to apply for an auto
loan with East West Banking Corporation (“EWBC”) to finance the purchase of a
motor vehicle (“Vehicle”) from ____________________________, for the benefit of
our family.

2. As part of EWBC’s documentation requirements for the auto loan, my

spouse and I are being required by EWBC to sign the requisite loan and mortgage
documents and other related documents as borrowers and mortgagors.

3. My spouse cannot sign any and all such documents because he is

currently out of the country. He is ( briefly
describe or explain the circumstances of the absent spouse being out of the country /
overseas Filipino worker).

4. My spouse has consented to and has given me his/her permission to

apply for such auto loan, get the loan proceeds to purchase the Vehicle for the benefit
of our family, secure the loan with a chattel mortgage on the Vehicle, and sign and
execute any and all documents for the loan and chattel mortgage.

5. Attached hereto and made an integral part hereof as Annex A is a

photocopy of our marriage contract. Likewise attached hereto and made an integral
part hereof as Annex B is the proof that my spouse is employed as an OFW at
________________________, in ____________________.

6. I am executing this affidavit to attest to the truth and correctness of the

foregoing statements.
