 INSTRUCTIONS:
• A separate License form should be filled for each weapon
• Form should be filled in English using capital letters only
• Use a ballpoint pe n with Black ink.
• Submit a photocopy of CEO a nd contact person’s CNIC with the application form
• Paste one photograph of company’s Monogram/Logo on the form and attach one with form. The
company’s Name must be written on backside of their re spective Monogram.
• Thumb Impression and signature should not exceed out of rectangle allocated for it.
• Name of company to be printed on the card should not exceed from 25 characters including space
• Column No.4,7,8,11to15 ,17to19 and 29 are mandatory.
• Column No.21to28 are mandatory for Renewal/Conversion application.
Attach following documents with the application:-
• Incorporation certificate issued by SECP.
• N.T.N. certificate.
• Form-29 duly certified by SECP
