JOINT-AFFIDAVIT OF DISINTERESTED PERSON

(Re: Discrepancy in the Name)

We, EVANGELINE Y. ALABADO and MARGARITA A. COQUILLA, both Filipinos,

of legal ages, both married, and residents of Barangay Trinidad, Calbayog City, Samar,
Philippines, after having been duly sworn to in accordance with law, hereby depose and
state, that--

1. We personally know FRANCISCO SINGARIOUS SALDAŃA of Barangay

Trinidad, Calbayog City, Samar, because during his lifetime, we have known
him in the community since we are neighbors and close family friends;

2. We know for a fact that FRANCISCO SINGARIOUS SALDAŃA is a legal

beneficiary of the government’s Unconditional Cash Transfer Program;

3. It was discovered in his UCT Beneficiary form that his Name was recorded
FRANCISCO MADRID SALDANA instead of FRANCISCO SINGARIOUS
SALDAŃA. The name MADRID as appearing in his UCT Form is the maiden
name of his mother Apolonia Madrid Singarious;

4. Amidst the foregoing error and discrepancy, we can attest to the fact that his
correct and complete name is FRANCISCO SINGARIOUS SALDAŃA as
reflected in his COMELEC Voter’s Registration and other personal records;

5. Under such circumstances, the record from the UCT Beneficiary Form and
COMELEC records refers to the same and one person FRANCISCO
SINGARIOUS SALDAŃA;

6. The foregoing discrepancy could be attributable to the excusable mistake and

negligence of its informant and or the person who prepared, recorded and
cause the same, and not intended to cause confusion;

7. We are not related to him or to his family members either by consanguinity or

affinity;

8. We are executing this Affidavit to attest to the truth of the foregoing facts and
for whatever legal purpose this may serve;

IN WITNESS WHEREOF, we have hereunto set our hands this 30th day of October
2018, at Calbayog City, Samar, Philippines.
