AFFIDAVIT OF ONE AND THE SAME ADDRESS

I, Ricarte G. Ganotisi, Jr., Filipino, of legal age, and a resident of

______________________________________________, hereby depose and
state under oath that:

1. I am the owner of the sole proprietorship enterprise with the

business name of Bialogvski General Merchandise;
2. The sole proprietorship mentioned in the immediately preceding
item is engaged as an import and export indentor;
3. The business address of Bialogvski General Merchandise is at No.
220, Phase III, Brgy. Burol, Dasmarinas City, Cavite;
4. The shipment of certain cargoes are being delivered directly to the
clients of the business;
5. The clients of the business designate the business address of the
sole proprietorship to be the area that will receive the merchandise
delivered;
6. The address of the sole proprietorship and the designated point of
delivery for the clients are one and the same;
7. I execute this affidavit to attest to the truth of the above statements
and for whatever lawful purpose it may serve.
