PETITION

COMES NOW, the Petitioner, and unto this Honorable Court, most respectfully
states, that:
1. She is a Filipino citizen, of legal age, born on March 10, 1993 at Clarc City,
Philippines and is presently residing at 123, Malvar, Davao, Philippines with residential
Phone Number (082) 216-0023 and Law Office Number (082) 227-911;
2. She has paid her professional tax for the current year in the City of Davao
City under Official Receipt No. 014344 issued on December 1, 2023;
3. She holds office located at Door No. 3, Blink Building, F. Torres St., Davao
City with telephone no. (082) 227-911;
4. She has satisfactorily completed the course in law in the College of Law of
the Jose Maria College in 2021;
5. She passed the Bar Examination in 2022 under Roll of Attorney’s No.
2016400024; Page No. 3; Book No. 10; and she is a member of the Integrated Bar of the
Philippines, Davao City Chapter, as evidenced by a copy of his Certification of
Membership in the Philippine Bar, issued by the Office of the Bar Confidant, Supreme
Court, and the same is attached hereto as Annex “A”;
6. She has been a member of good standing of the Integrated Bar of the
Philippines, Davao City Chapter, as evidenced by a Certification issued by the IBP
Davao City Chapter attached hereto as Annex “B”;
 7. She possesses all the qualifications and none of the disqualifications for
the office of a notary public;
8. She has never been convicted of any crime involving moral turpitude,
neither does she have any pending administrative case against her and this fact is
evidenced by a Certification from the Office of the Bar Confidant, Supreme Court, and
the same is attached hereto as Annex “C”;
9. She is attaching the following as:
Annex “D” – a sheet of bond paper bearing three (3) specimen of her official
manual signature and three (3) impressions of her official dry seal pursuant to law;
Annex “E” – a sheet of bond paper to which three (3) passport size pictures are
attached;
Annex “F” – a machine copy of her Professional Tax Receipt with No. 014344
issued on December 1, 2023 at Davao City, Philippines;
Annex “G” – a machine copy of her IBP Official Receipt No. 09338161 issued on
December 11, 2023 by IBP Manila;
Annex “H” – a machine copy of her receipt for the purchase of her Notarial Seal
purchased November 20, 2023.

WHEREFORE, it is respectfully prayed of this Honorable Court that an order be

issued re-appointing the undersigned petitioner as a Notary Public for and in the City
of Davao.

Petitioner prays for such other remedies as this Honorable Court may deem just
and equitable in the premises.
