AFFIDAVIT OF LOSS

I, JESIEBEL D. ARNADO, of legal age, Filipino, single, and a resident of Prk. Durian, Tagum
City, Davao del Norte, Philippines, after having been duly sworn to in accordance with law, do hereby
depose and say:

1. That I am the credit investigator of SHELAMAR LENDING CORPORATION;

2. That I am responsible for the conduct of interview and investigation of the borrower-
applicant’s personal circumstances, source/s of income and other matters relevant for
approval of loan applications;

3. That Mrs. Ana Osman, of legal age, Filipino, married and a resident of Prk. 7, Lower
Apokon, Tagum City, one of the borrower-applicants submitted her documentary
requirements to our office including her VOTER’s ID;

4. That the said requirements including the VOTER’s ID were forwarded to me for the conduct
of the requisite interview and investigation;

5. That the said VOTER’s ID, while conducting ocular inspection, was lost and despite exerting
diligent efforts to locate it, the same could no longer be located;

6. That I am executing this affidavit to attest to the truth of the foregoing facts and for any
legal purpose this may serve best.
