1. That I am a member of the Philippine Natonal Police since May

2006, with badge number 168766 and duly assigned at Community
Police Assisstancve Center 2 of Cuty of San Fernando Pampanga
wherein Iam destinated as patrolletr;

2. That I have executed a reply-affidavit regarding the incident that

happened last October 21, 2018 wherein I was caught sleeping by
the inspecting team of RIAS 3;

3. That I confirm and affirm all contents and truthfulness of the said
reply affidavit;
