C. LOCATOR’S CERTIFICATION AND GUARANTEE

We hereby jointly bind ourselves to observe all the rules and regulations of the Cavite Economic Zone and
those stated at the back of this form. Upon sale or disposal of my vehicle, we commit to remove the vehicle sticker
and return the same to CEZ Police/PVICU office.
We hereby further bind ourselves to the following:
1. That the sticker remains the property of CEZ.
2. That any misdeclaration of information and misappropriation of the CEZ sticker shall be ground for
the revocation without prejudice to whatever sanction allowed by law and/or CEZ rules.
We hereby further certify that all information declared herein is true and correct .

PROCEDURES/GUIDELINES FOR APPLICATION AND USE OF

CEZ SECURITY VEHICLE STICKER

1. Present duly accomplished Application Form together with photocopies of the following:
a. Photocopy of Vehicle’s current LTO Official Receipt (OR)
b. Photocopy of Vehicle’s Certificate of Registration (CR)
c. Photocopy of Valid Driver’s License of Vehicle Owner or Authorized Driver
d. Proof of Employment (e.g. Certificate of Employment) for Employees
e. Certificate of being a Supplier for Company Suppliers.

2. If the vehicle is not registered under applicant’s name, present also duly notarized Deed
of Sale or Authority to use from registered owner, marriage contract for spouses, and birth
certificate for relationship basis with the owner. PEZA reserves the right to disapprove
applications if vehicle is found to be inappropriate to use CEZ security vehicle sticker.

3. Only the CEZ authorized representative shall install the sticker on applicant’s vehicle.

4. CEZ security stickers are NON-TRANSFERABLE.

5. Sticker installed by temporary means such as scotch tape will automatically be confiscated
by the Cavite Economic Zone Security.

6. Violation of Rules nos. 4 & 5 carry a penalty of P1, 000 to be paid upon request for re-
issuance or renewal of the CEZ security vehicle sticker.

7. REGISTRATION FEES

 LOCATOR & EMPLOYEE VEHICLES

 Cars and motorcycles : Pesos 100.00
 Trucks (Six Wheels & Up) : Pesos 1,570.00

 NON CEZ VEHICLES (Companies and entities whose business address/es are outside
the Cavite Economic Zone like banks, forwarders, canteen operators, scrappers,
suppliers, service providers, etc.)
 Cars and motorcycle : Pesos 400.00
 Trucks (Six Wheels & Up) : Pesos 1,570.00

8. The vehicle’s owner shall be equally and directly liable for all violations of the CEZ rules and
regulations committed by his/her authorized driver.

9. The issuance of this vehicle sticker does not carry any acceptance of liability on the part of
CEZ.

10. All vehicles are subject to the CEZ rules and regulations.

11. The Cavite Economic Zone reserves the right to refuse issuance of its security vehicle
stickers to applicants who are considered security risks to the general welfare of the Cavite
Economic Zone and its locator-member firms.

12. The security vehicle sticker is valid for one year and renewable thereafter at the sole
discretion of the CEZ Administration.
