AFFIDAVIT OF BEING SINGLE

I, JOEMAR M. SANG-AN, of legal age, single, Filipino and a resident of

Purok 2-Poblacion, Prosperidad, Agusan del Sur, after having been duly sworn to
in accordance with law, hereby depose and say:

That I am applying in the Military Service as trainee in the

Philippine Army;

That I was born on October 1, 1994 and one of the children

of spouses MAGNO SANG-AN and DAMIANA SANG-AN;

That I am still single, as I have never contracted any

marriage with another person up to the present;

That I am executing this affidavit to attest to the truth of the

above-statement for whatever legal purposes that this affidavit it
may serve.
