CONTENT VALIDATION FORM

To the Content Validator:

Kindly use this form in validating the quarterly test in Mathematics Grade 7 constructed by CTP 7-1 Group 4 considering
the following criteria:

a. The test items are aligned with the objectives specified in the Table of
3 Specifications (TOS).
b. The test items are appropriate to Anderson and Krathwohl’s Taxonomy of the

c. The test items are clear and considered the guidelines in test construction.
The constructed test NEEDS REVISION. This means that it did not satisfy one or

The constructed test is NOT ACCEPTABLE. This means that it did not satisfy ALL of

the above conditions. Kindly specify the reasons of non-acceptability.
The format of the test is ACCEPTABLE. This means that it meets the following
conditions:
3 a. There are no mechanical errors (ex: alignment, spacing, spelling, etc.).
b. The font type, size, and spacing used are reader-friendly.

READABILITY
Example: Content and Language are both acceptable. However,

The Content, Format & Readability and Language are all
3 acceptable. It’s just that maybe you can make the blanks

before the number a little bit longer to provide adequate
space for the answers.

acceptable. It’s just the content that we have a problem. I

think the items for Multiple Choice is quite many and
quite hard to answer for a Grade 7 student.
The Format & Readability is acceptable. It’s just that the
content is quite lacking and the language is quite
confusing. Maybe you should elaborate more the

direction. You should put Roman numeral IV on the last
part of the test since it’s different from the Multiple
Choice.
