A
Ortopedia e Traumatologia é uma especialidade que cres-
ce numa velocidade muito grande, e acompanhar seus

avanços tornou-se uma tarefa difícil. Ao médico cabe a
responsabilidade de tentar seguir todos os progressos, aplicando-
os aos seus pacientes e divulgando-os. Hoje, a ortopedia é a
especialidade mãe que gerou uma série de outras, que também
crescem de forma muito rápida. A importância de proporcionarmos
ao ser humano o melhor atendimento médico, torna fundamental a
atualização e a reciclagem dos profissionais.
Por esta razão, decidimos unir esforços e produzir fascículos
periódicos abordando assuntos que consideramos muito importan-
tes. O primeiro trata dos reimplantes dos membros. Seguramente
todo médico irá deparar-se com pacientes vítimas de amputações
traumáticas. É mister saber conduzir adequadamente o tratamen-
to, que tem por objetivo, o restabelecimento da anatomia e da fun-
ção. Os procedimentos adotados na fase aguda do trauma pode-
rão significar o retorno do paciente a uma vida produtiva, ou uma
seqüela definitiva. Para adotar condutas corretas é preciso reunir
conhecimentos básicos, que procuraremos enfatizar com texto e
ilustrações cuidadosamente produzidas para a classe médica. O
ideal é que cada profissional da saúde atue na educação da popu-
lação, prevenindo os acidentes, e orientando a conduta frente a
uma amputação traumática.

DEFINIÇÃO
REIMPLANTE
Procedimento cirúrgico de re-
construção das artérias e veias, e das
demais estruturas, de um segmento
amputado, de forma completa. O
objetivo do reimplante não é apenas
restabelecer a perfusão sangüínea,
mas obter o retorno da função da
extremidade.

TÉCNICA CIRÚRGICA
1. LIMPEZA DOS FERIMENTOS:
os cotos distal e proximal são lavados
exaustivamente com soro fisiológico,
utilizando solução antiséptica no tegu-
mento cutâneo íntegro . Para evitar
sangramento, no coto proximal utiliza-
mos um torniquete pneumático
2. DESBRIDAMENTO E DISSEC-
ÇÃO DAS ESTRUTURAS: as inci-
sões nos cotos proximal e distal são
planejadas de acordo com o tipo de
ferimento. Basicamente, procuramos
criar dois retalhos, um volar e outro
dorsal. Na grande maioria das vezes
realizamos incisões médio-laterais ou
em múltiplos "Z. Ao nível de antebra-
ço, punho, mão e dedos, através do
levantamento do retalho volar tere-
mos acesso à artéria, nervos e estru-
turas músculo-tendíneas flexoras. O
levantamento do retalho dorsal pro-
porciona acesso às veias e estruturas
músculo-tendíneas extensoras. Todo
tecido desvitalizado e contaminado
deve ser ressecado, tomando-se o
cuidado de utilizar lentes de magnifi-
cação. Devemos lembrar que o tecido
desvitalizado é a principal causa de
infecção.
Com o desbridamento procura-se
tornar uma amputação provocada por
mecanismo de esmagamento ou avul-
são em uma provocada por mecanis-
mo inciso tipo guilhotina.
3. SEQÜÊNCIA DA RECONS-
TRUÇÃO: se não houver tempo de
isquemia crítico, o cirurgião poderá
escolher a melhor estratégia para
reconstrução das estruturas. A
seqüência utilizada com maior fre-
qüência é a reconstrução óssea
(encurtamento ou regularização com
fixação), seguida de reconstrução
dos tendões extensores, anastomose
das veias dorsais, sutura da pele dor-
sal, tenorrafia dos flexores, anasto-
mose das artérias, neurorrafias e
sutura da pele volar. Em algumas
situações, fazer a anastomose arte-
rial antes da venosa é vantajoso pois

4. ENCURTAMENTO ÓSSEO E
OSTEOSSÍNTESE: após o desbrida-
mento de todas estruturas, o osso
deve ser encurtado e fixado. O encur-
tamento é realizado para promover a
fixação entre extremidades regulares,
limpas e viáveis. Este procedimento
melhora o índice de sucesso quanto à
consolidação óssea e diminui a ten-
são das outras estruturas a serem
reconstruídas, como vasos, nervos e
tendões. O tipo de osteossíntese a
ser escolhido dependerá das condi-
ções do tecido ósseo e da localização
da lesão. Deve-se sempre escolher a
osteossíntese mais eficiente, com o
objetivo de proporcionar movimenta-
ção articular precoce, e a mais sim-
ples e rápida possível. Entre as
opções mais utilizadas estão as pla-
cas de impacção, fios de Kirschner,
fixadores externos, amarrilhas e ban-
das de tensão. As lesões que com-
prometem uma articulação podem ser
tratadas com uma artrodese primária.

5. ANASTOMOSE VASCULAR:
normalmente as anastomoses vascula-
res são realizadas concomitantemente
após a reconstrução dos tendões flexo-
res e extensores. As microanastomo-
ses são realizadas com auxílio do
microscópio cirúrgico, instrumental e
fios de microcirurgia. A técnica de
microanastomose vascular deve ser
aprendida e treinada em laboratórios de
microcirurgia. Para procedimentos de
reimplantes normalmente utilizamos
anastomoses término-terminais de
artérias e veias. Freqüentemente, após
o desbridamento há necessidade de se
utilizar enxertos vasculares para
reconstrução da perda segmentar dos
vasos. Pode-se utilizar heparina tópica
para diminuir o índice de trombose e
xilocaina sem vasoconstrictor para evi-
tar o espasmo. Para evitar as trombo-
ses da microanastomoses o vaso deve
ser desbridado de forma que a sutura
deve ser realizada entre vasos normais.
Freqüentemente, após o desbridamen-
to há necessidade de se utilizar enxer-
tos vasculares para reconstrução da
perda segmentar dos vasos.

6. ANASTOMOSE NERVOSA: os
nervos são, geralmente, as últimas
estruturas a serem reconstruídas. O
sucesso da função do reimplante esta
intimamente relacionada com a quali-
dade da reconstrução dos nervos
periféricos. A técnica de reconstrução
do nervo periférico dependerá do ner-
vo e do local envolvido. Usualmente
os nervos digitais, que são oligofasci-
culares e puramente sensitivos, são
reconstruídos por meio de sutura epi-
neural externa. Os nervos mediano e
ulnar, que possuem uma estrutura
fascicular organizada, podem ser
reconstruídos pela sutura epineural
interna. Muitas vezes, após o desbri-
damento, há uma perda segmentar do
nervo periférico que exige reparação
com enxertos de nervo.
