father MUHAMMAD ASIF, hereby bind ourselves, our heirs, executors and administrators jointly and
severally to be Government of the Punjab for the payment of a sum of Rs. 30,00,000/- (Rupees Thirty Lac
Only) on demand.
Whereas the above bonded IQRA ASIF admitted in 1st year MBBS class in Khawaja Muhammad Safdar
Medical College, Sialkot on a clear undertaking by him that he would join Government Services in the Punjab
or KMSMC and serve for a period not less than one year immediately after the MBBS Degree in conferred on
him.
And whereas the same IQRA ASIF and the said MUHAMMAD ASIF the above bond in the sum or Rs.
100000/- (Rupees One Lac) conditioned for the due performance by IQRA ASIF of the said undertaking.
Government of the Punjab or KMSMC or offer himself/ herself to join such service as Medical Officer
immediately after the conferment of the MBBS Degree on him and shall not quit such service before the
completion of the three years than the above written bond shall be void and of no affect, otherwise it shal
remain in full force and virtue.

Reference your application for admission in M.B.B.S course for the academic year 2018-19. You have
been selected for Admission in the first year M.B.B.S class for the Academic Year 2018-19 at Govt. Khawaja Muhammad
Safdar Medical College, Sialkot on Provisional basis subject to the following terms & conditions:-

a). You have to submit following documents in original and eight (08) attested copies of each document to
the College Office before depositing the College fee:-
1. Secondary School Certificate (Matriculation Certificate).
2. Certificate of the O-Level.
3. Equivalence Certificate of the O-Level.
4. Certificate/ Detailed Marks Certificate of F.Sc (Premedical).
5. Certificate of the A-Level.
6. Equivalence Certificate of the A-Level.
7. Entry Test Result Card.
8. Provisional Certificate/ Character Certificate of Last College attended.
9. Original Domicile Certificate.
10. Board Migration Certificate in Case of the F.Sc other than Gujranwala and Lahore Boards.
11. Sixteen Latest Passport photographs {3 x 3cm (8 Nos.), 2.00 x 2.5cm (8 Nos.)} (Sky-blue
Background) duly attested (Backside) By Government officer (Grade-17 or above).
12. Certificate of Vaccination of Tetanus and Hepatitis “B” (specimen available on college website).
13. Student’s N.I.C. or B- form copy.
14. Father/ Guardian’s N.I.C. Copy.
15. Surety Bond (specimen available on college website) on a stamp paper of worth Rs. 100/- duly attested
by the Civil Judge (specimen available on college website).
b). After submission of the above mentioned documents, you will submit the College fee as per following:-
1. College Fee: Rs.32,300 /-

c). Your admission is liable to be cancelled at any time during the course of study if any document,
certificate or statement is found wrong in any way at any stage. Admission is also be cancelled/ changed if any error or
omission is detected at any stage.
d). Apart from this, all the rules and regulations contained in the prospectus of (University of Health
Sciences, Lahore) Medical Colleges of the Punjab and changes from time to time are applicable and they are binding on
you.
e). If you accept, please report to the office of the undersigned along with this letter, fee and documents
mentioned above on or before 22-11-2018. If you fail to pay the fee along with required documents and certificates
within the stipulated date, your selection will be cancelled. In case you are not willing to accept these terms and
conditions, please submit your reply in writing to this office within the stipulated time. The date of start of classes will
