Ringkasan Kejadian:

Pada tanggal 3 Juni 2011, sekitar pukul 02.30 pagi, seorang karyawan pihak ketiga tewas setelah terlindas oleh
dua Front End Loaders (FEL). Korban bekerja sebagai pengemudi truk, korban saat itu sedang mengirimkan fly
ash basah ke gudang bagian operasional. Sekitar pukul 1.00 pagi, korban memarkir truknya untuk melakukan
pembongkaran muatan di gudang tersebut. Sementara truk tersebut dibongkar muatannya, korban
memutuskan untuk istirahat/tidur di lantai dekat bagian depan truknya. Lokasi di mana korban tidur ternyata
merupakan jalan masuk ke area gudang gypsum. Kecelakaan terjadi pada saat 2 Loader yang dioperasikan di
area gudang gypsum dan clinker melewati akses jalan masuk tersebut. Korban berbaring di jalur akses loader
dan kemudian terlindas oleh kedua loader; operator loader tidak melihat korban pada saat mereka melewati
lokasi tersebut.

INTI PEMBELAJARAN

1. Cara kerja aman harus diiikuti untuk pengoperasian kendaraan dan alat berat di lokasi pemuatan dan
pembongkaran, diantaranya adalah:
o Penerangan yang cukup untuk lokasi yang diperuntukkan sebagai jalan kendaraan dan
alat berat,
o Menyediakan dan memastikan setiap orang memakai pakaian dengan warna terang/
mencolok (high visibile) pada saat bekerja atau berada di sekitar kendaraan atau alat
berat.
o Pembatasan orang untuk bertemu langsung dengan kendaraan yang bergerak dan lalu
lintas, termasuk memastikan pengemudi beristirahat di lokasi aman yang disediakan.
2. Kurangi paparan resiko orang untuk keluar dari kabin di lokasi kerja dan meningkatkan efisiensi
pemuatan dan pembongkaran material dengan metode/alat bantu mekanis (truk dengan bak ungkit,
ekskavator, dll)

Summary of Case:

On 3rd June 2011, at about 2.30 am, an employee from a third party service provider was fatally injured as a
result of being run over by two Front End Loaders (FEL). The victim was employed as a truck driver, he was
delivering wet fly ash to the operations storage facility. At approximately 1.00am the victim has
parked/positioned his truck to be unloaded at the storage facility. Whilst his truck is being unloaded the victim
has decided rest/sleep on the ground near the front left side of his truck. The location where the victim has been
resting/ sleeping was also entrance to the gypsum storage area. The incident occurred when two FEL which
were operating within the gypsum and clinker storage areas drove through this entrance/access point. The
victim was lying in the path of travel of the FEL and has subsequently been run over by both loaders; neither
loader operator saw the victim on the ground as they travelled through this location.

KEY LEARNING POINT

1. Safe working procedures must be followed for the operation of vehicles and heavy mobile equipment in
loading and unloading areas, such as:
o Appropriate illumination/ lighting arrangements at locations which are subject to
movement of vehicle and mobile equipment ,
o Provide and ensure personnel wear high visibility clothing when they are required to work/
in and around vehicles and heavy mobile equipment.
o Restricting / limiting the personnel from interfacing with moving traffic and equipment, this
includes ensuring drivers and their assistants rest at designated safe areas which have
been provided.
2. Reduce the risk exposure of personnel exiting their vehicles in operational areas by and increase the
efficiency of loading and unloading arrangements through mechanized methods (trucks fitted with tip
trays; excavator etc)
