Seseorang tewas akibat kecelakaan...

Pada tanggal 3 Agustus 2012, sekitar pukul 6:00 sore, di Jalan tol BR 040, seorang karyawan kontraktor pihak
ketiga tewas setelah mengalami kecelakaan lalu lintas. Saat itu korban sedang mengangkut semen bulk dari
Pabrik Barroso menuju lokasi konsumen di kota Rio de Janeiro, yang berjarak sekitar 300 km dari Pabrik.
Menurut laporan awal dari tenaga ahli, truk saat itu memasuki sebuah tikungan dan jalan yang menurun dengan
kecepatan tinggi, menabrak pagar pembatas di tengah, terbalik dan terseret sejauh 100 m di jalur yang
berlawanan arah sebelum akhirnya bertabrakan dengan truk lain yang melaju dari arah yang berlawanan. Supir
truk (korban) langsung tewas di tempat kejadian. Sedangkan supir truk yang satunya lagi langsung dibawa ke
Rumah Sakit dengan cedera ringan di kepala. Menurut laporan awal tenaga ahli, truk kehilangan kendali akibat
kegagalan dalam pengereman.

Inti Pembelajaran
Jika memungkinkan, pertimbangkan hal-hal berikut ini:

• Tekankan pentingnya “preventive maintenance”, khususnya pada sistem pengereman

• Lakukan penyegaran mengenai kesadaran/pengetahuan terkait cara pengereman yang tepat di jalan yang
menurun. Termasuk tindakan yang diambil jika terjadi kondisi darurat, seperti kegagalan pada sistem
pengereman;
• Untuk informasi lebih lanjut mengenai pengoperasian aman kendaraan, silahkan merujuk pada FPE3
Vehicle and Traffic Safety.

A Person was fatally injured…

On August 03th, around 6:00 p.m., on Highway BR 040, an employee of a third party service provider with
Holcim’s semi-trailer was fatally injured in a traffic accident. He was transporting bulk cement from Barroso
cement plant to a customer in the city of Rio de Janeiro, located approximately 300 km from the plant.
According to the preliminary expert report, the truck entered in a curve on a declining road at high speed, crashed
into the center guardrail, overturned and dragged for about 100 meters in the opposite direction before colliding
with another truck. The truck driver passed away at the accident scene. The other truck driver involved in the
accident was taken to the local hospital with minor head injuries. Based on the preliminary expert report, the
truck lost control due to a brake failure.

Key Learnings
If applicable, please consider the following aspects:

• Emphasize the importance of preventive maintenance, especially of the braking system

• Refresh the awareness of proper brake usage on roads with slopes. As well as reaction in case of
emergency situations, i.e. in case of brake failure;

Prevention Element FPE 03 (Vehicle and Traffic Safety).
