Seseorang tewas akibat kecelakaan...

Pada tanggal 17 Agustus 2012, antara pukul 11:10 s/d 11:45 siang, seorang karyawan menutup frame atas ”5263
Telsmith Horizontal impact crusher” (HAC 3986.321-HI01) di Plant Recycled Asphalt (RAP) Sloan, 15 mil barat daya
Las Vegas. Mengikuti prosedur yang ada, karyawan tersebut menghidupkan system hidrolik, mengangkat frame
bagian atas tersebut sehingga dia bisa melepas batang penyangga yang dipasang selama proses
perawatan/perbaikan dilakukan, kemudian melepaskan sistem hidrolik dan memasang pengunci frame tersebut.
Untuk alasan yang tidak diketahui, setelah dia melepas batang penyangga, dia memasuki crusher. Pada saat
korban berada di dalam crusher, kerangka bagian atas tiba-tiba menutup – sistem hidrolik tidak bekerja menahan
bagian atas kerangka – dan menjepit korban. Sebuah perkakas (soket besar) ditemukan terjepit di antara frame
bagian atas dan bawah. Kemungkinan korban masuk ke dalam crusher untuk mengambil soket tersebut.

Inti Pembelajaran
Jika memungkinkan, pertimbangkan hal-hal berikut ini:

• Seluruh elemen LOTO (FPE2) harus diikuti: “jangan mengandalkan tekanan hidrolik untuk menjaga frame
tetap terbuka.” Pada saat penyangga/penahan/pengaman dilepas, area tersebut “TIDAK BOLEH
DIMASUKI” dan memasuki area tersebut dengan tegas dilarang.
• Pembersihan setelah pekerjaan perawatan/perbaikan harus benar-benar selesai sebelum alat pengaman
dilepas dan penutupan alat dilakukan. “Pekerjaan belum selesai sampai dengan pembersihan selesai
dilakukan!”
• Setelah pekerjaan dimulai, jika ada perubahan cakupan pekerjaan – hentikan pekerjaan dan evaluasi
kembali Analisa Kerja Aman (JSA).
• Pada saat memilih peralatan yang sama, pertimbangkan keterbatasan akses dan ruang.

A Person was fatally injured…

On August 17, 2012 between 11:10 and 11:45 AM, an employee was closing the top frame on a 5263 Telsmith
Horizontal impact crusher (HAC 3986.321-HI01) at the Sloan Recycled Asphalt (RAP) plant at Sloan, 15 miles south
west of Las Vegas. Following required procedures, the employee energized the hydraulic circuit, lifted the frame to
a position at which he could remove the supporting brace which was kept in place during the maintenance process,
then de-energized and again locked out the equipment. For reasons unknown, once he had removed the brace, the
employee entered the crusher. While the employee was inside the crusher, the top frame closed suddenly – the
hydraulic cylinders did not hold it open - pinning the employee. A tool (large socket) was found wedged between
the top and bottom frames. The employee may have entered the crusher to retrieve this socket.

Key Learnings
If applicable, please consider the following aspects:

• All elements of Lock-Out-Tag-Out (FPE#2) must be followed: per task training “do not rely on hydraulic
pressure to keep the frame open.” When a safety support is removed, the area is a “NO GO” area and
entrance is strictly forbidden.
• Clean up after maintenance jobs must be fully completed before safety devices are removed and final
equipment close up is initiated. The “job is not done until the clean-up is done!”
• After a job begins, if the scope changes – stop and reevaluate the Job Safety Analysis.
• When selecting new similar equipment consider relevant access and space limitations.
