Inti Pembelajaran
Jika memungkinkan, pertimbangkan hal-hal berikut ini:
• Pastikan pengaturan lalu lintas di dalam Plant mencantumkan perintah yang jelas “Dilarang
Masuk”untuk area-area tertentu (contoh: area lalu lintas kendaraan berat).
• Lakukan pemisahan antara jalur pejalan kaki dan jalur alat berat, gunakan rambu yang sesuai.
• Tinjau ulang aturan mengenai kaca spion untuk menghindari area blind spots dan pastikan tidak
ada yang mendekati kendaraan/alat berat sebelum melakukan komunikasi yang jelas dengan
operator/supir alat berat/kendaraan tersebut.
• Tinjau kembali langkah pengendalian yang ada apakah bekerja dengan baik atau tidak, dengan
melakukan inspeksi, audit dan SOT secara sistematis.

A Person Died...
A Person was hit by a loader when entering a wash truck area
On August 14th 2013, at 14:15 a truck of a third party service provider entered the Puente Maipo Plant
to load sand to be delivered to a customer. The driver of the truck was instructed by the dispatch office
to go to the sand pile where he would to be loaded with this material.
The driver decided to go to the truck wash area and wash his trailer before loading. After pressing the
start bottom of the truck wash, for unknown reasons the driver walked in the roadway that is used by
heavy mobile equipment. He was hit by a third party plant loader transporting aggregates on the
roadway as the driver of the loader could not see him.
Plant personnel called immediately the medical center and the ambulance arrived a few minutes later,
but the worker died before the arrival of the ambulance as a result of his injuries.
Key Learnings
If applicable, please consider the following aspects:
• Ensure traffic management plans with clear instructions on No Go zones are in place and
regularly updated.
• Implement segregation of pedestrians and heavy vehicles as well as respective signage.
• Review the provision of mirrors to avoid blind spots and make sure no person approaches a
Vehicle without first making appropriate contact with the operator/driver of that equipment.
• Review the controls are working through systematic inspections, audits and safety observation
tours.
