Satu orang tewas akibat kecelakaan…

Seorang karyawan pihak ketiga tertabrak alat pemuat semen untuk gerbong kereta
pada saat memuat semen …
Pada tanggal 29 Maret 2013, pukul 12.50 waktu setempat, seorang karyawan kontraktor pihak ketiga yang sedang
bekerja memuat semen di dalam gerbong kereta tertabrak alat pemuat semen, yang dioperasikan rekan kerjanya
yang lain yang juga di dalam gerbong. Saat itu proses pemuatan sudah mendekati bagian pintu gerbong.
Kecelakaan terjadi di bagian ujung alat pemuat semen, pada bagian yang terdiri dari conveyor teleskopik dengan
plat besi diujungnya (silahkan lihat gambar). Bagian ini bisa digerakkan dan disesuaikan menurut ketinggian dan
dapat bergerak maju mundur, dimana gerakan ini dikendalikan oleh operator melaui panel kontrol. Sedangkan
untuk gerakan berputar/menyamping dikendalikan secara manual oleh petugas pemuatan.
Korban tertabrak oleh plat besi di ujung conveyor teleskopik ini pada saat bagian ini bergerak maju, korban
terjepit antara tumpukan semen dan plat besi dari conveyor. Korban segera dibawa ke rumah sakit perusahaan,
dimana petugas paramedis memberikan pertolongan pertama. Kemudian korban dirujuk ke rumah sakit yang
lebih lengkap dimana korban dinyatakan meninggal pada saat pemeriksaan.

Inti Pembelajaran
Jika memungkinkan, pertimbangkan hal-hal berikut ini:
• Pastikan resiko terjepit peralatan bergerak sudah diidentifikasi di SOP dan langkah pengendalian sudah
dilakukan, khususnya:
o Pekerja tidak boleh berada di lintasan alat pemuatan dan selalu berada di jarak aman dengan ujung plat
besi mesin pemuat.
o Sarana dan prosedure koordinasi dan komunikasi antar pekerja harus selalu memastikan untuk selalu
menjaga jarak aman.
• Pekerja harus dilatih dan kompeten terkait dengan bahaya aktifitas pemuatan dan penumpukan semen.
• Alat penghenti darurat harus bisa dicapai dengan mudah oleh petugas pemuat dan jika memungkinkan,
pasang sistem penghenti otomatis jika terjadi benturan depan antara plat besi dengan orang.

A person died…
An employee of a third party service provider was struck by the train wagon loader
during cement bag Loading…
On 29th March 2013, at 12.50 hrs, an employee of a third party service provider was hit by the train wagon loader,
operated by his co-worker (wagon loader operator) inside a train wagon, whilst stacking of cement bags was in
progress in the door corridor section.
The incident happened at the discharge section of the train wagon loader machine consisting of a telescopic
conveyor section with the lip plate at the end (refer photograph). This section is adjustable in height and moves
forward and both these motions are controlled by operator through control panel while the radial movement on
either side is controlled manually by the loader.
The loader was struck by the lip plate of the telescopic section during its forward movement and got trapped
between the bag stack on either side (congested area) and the lip plate of the loader conveyor. The injured
employee was immediately shifted to the works hospital, where initial treatment was provided by the works
Medical Officer. Then, he was shifted to a higher level hospital where he died during examination.

Key Learnings
If applicable, please consider the following aspects:

• Ensure all risks of entrapment with moving machinery are identified in the safety operational procedure (SOP)
and control measures are in place. especially:
o Workers should not be positioned in in the trajectory of the wagon loader and a safe distance from the
edge of the lip plate of the loader must be maintained.
o Coordination and communication tools / procedures between employees must allow that safe working
distance may be maintained.
• People must be competent and trained with regard to operational hazards of loading and stacking operations.
• An emergency stop switch needs to be easily accessible to the loader and also evaluate installing auto stop
mechanism in case of frontal impact of person on the lip plate.
