* Benadryl: 25-50mg/ 50 mL 0.9% NSS IV

* Calcium Gluconate: 10 mL/ 100mL 0.9% NSS IV to run in 1hr

* Cardizem (Diltiazem): Bolus 15-25 mg IV push. 100-125 mg/ 100mL D5W or 0.9% NSS IV pump @
5mL/hr

* Cordarone (Amiodarone): 150mg/100mL D5W IV to run in 10min, then 450mg/250mL D5W to

run at 33mL/ hr x 6 hrs, then to run at 16.6mL/hr x18hrs

* Digoxin: 0.5mg IV x 1dose then 0.25mg IV x 2 dose q 8hrs or 0.25 mg/50 mL 0.9% NSS IV to run in
1hr then 0.125mg PO qd

* Dilantin: 10-15mg/kg IV no faster than 50mg/min in Status Epilepticus. Loading dose 300mg/
250mL 0.9% NSS IV to run in 1-2 hrs then 100mg IV or PO q 8hrs

* Dobutrex (Dobutamine): 250mg/ 250mL D5W at 8 mL/hr or 500mg/ 250mL D5W at 10mL/hr

* Glucagon: 5-10mg IV in 5 min

* Heparin: Loading 80units/kg IV

then 2500 units/ 250mL D5W at 18mL/hr

* Insulin Pump: 100units/100 mL 0.9% NSS at 4mL/hr by IV pump (Dextro q 1hr)

100units/250mL 0.9% NSS at 6 mL/hr by IV Pump (Dextro q 2hrs)

0.1ml/kg is Loading Dose

* Intropin: 400mg/ 250mL D5W IV to run at 10-15mL/hr. 800mg/ 250 mL D5W IV (start running at
half of

what the previous was running)

* K3PO4:

PO4<1: 10mL/250mL D5W or 0.45% SS IV to run in 4hrs

PO4>1: 5mL/250mL D5W or 0.45% SS IV to run in 4hrsp

* KCL replacement: 40meq/ 500mL 0.45% SS IV to run in 8hrs or 20meq/250mL 0.9% NSS IV to run
in 6hrs
* Lasix Pump: 100mg/100mL 0.9% NSS IV pump at 5mL/hr

* Levophed: 4mg/250mL D5W IV pump to run at 4mL/ hr

* Lidocaine: 1g/250mL D5W IV at 15mL/hr. Bolus: 1mg/kg IV (60 mg IV bolus) for V tach

* Lovenox: ACS, angina, PE, MI, Ischemic CVA: 1mg/kg SQ q 12hr

DVT prophylaxis: 40mg SQ daily

CHF 40mg SQ daily

Orthopedic: 30mg SQ daily

Antidote: protamine sulfate 1mg/mg Lovenox or 1mg/100U

* Magnesium Sulfate (MgSO4):

1-2g/100mL 0.9% NSS to run in 1hr or 500mg IM in each buttocks

* Mannitol: 20%/250mL D5W IV to run in 1hr then 125mL IV q8hr

* NaHCO3:

2amp/1000mL 0.45% SS IV to run in 6 hr

1amp/1000mL 0.45% SS IV to run in 6 hr

* Narcan: 2mg IV push q2-3min for overdose with opioid, morphine, codeine, fentanyl,
meperidine, oxycodone

* Nipride: 50mg/250mL D5W IV to run at 3mL/hr

* Octreotide (Sandostatin): Loading Dose- 50-100mcg IV Stat

Then, 500mcg/100mL 0.9%NSS by IV pump to run at 10mL/hr or 1200mcg/250mL D5W IV pump to

run at 11mL/hr

* Prevacid: 30mg/50mL 0.9% NSS to run in 30min q 12hr

* Protonix: 80mg/100mL 0.9% NSS IV pump to run in10mL/hr

* Reglan: 5-8mg IV q8hr in a piggy bag or 10mg PO/IV q6hr or 30 min before meals TID

* Romazicon (Flumazenil): 0.2mg IV for BZD overdose (clonazepam,

diazepam, lorazepam, temazepam

alprazolam)
* Tridil: 50mg/250ml D5W IV to run @3ml/hr

* Vancomycin renal dose: 1g/250 mL NSS to run in 2hr x 1

* Vasotec: 1.25, 2.5, 5mg IV q6-8hr

* Versed: 50mg/100mL 0.9% NSS

by IV pump to run at 5mL/hr

* Vitamin K: 10mg/50mL D5W IV to run in 30min q8hr x3 for prolonged INR

* Zantac: 150-300mg/250mL D5W or NSS IV at 11mL/hr
