AFFIDAVIT OF ILLEGITIMACY

I, JUANA DELA KRUZ, of legal age, single, Filipino citizen and a

resident of AD-009 Central Buyagan, La Trinidad, Benguet, Philippines,
hereby depose that:

1. I am a mother of PETER BORHA who was born in Baguio City on

December 2, 2018 as evidenced by the attached Certificate of Live
Birth issued by the City Civil Registrar of Baguio City, referred to as
“ANNEX A”;

2. My son was born out of wedlock;

3. I am not legally married to his father who I no longer have any contact
with;

4. I am executing this affidavit to attest the above facts and to confirm

the ILLEGITIMATE STATUS of my son;

5. I am further executing this affidavit to comply with the regulations of

the Philippine authorities in connection with his travel to Hongkong
for all legal intents and purposes beneficial to my son.

IN WITNESS WHEREOF, I affix my signature on this 1 st day of April

2019, at Baguio City, Philippines, attesting to the truthfulness and veracity
of the foregoing for all legal intents and purposes it may serve.
