Asuhan Keperawatan Keluarga Tn. M Dengan Gangguan Sistem

Kardiovaskuler : Hipertensi di Puskesmas Sukalaksana Tasikmalaya

Karya tulis ini dilatar belakangi oleh ketertarikan penulis nutuk melakukan asuhan
keperawatan Keluarga Tn. M dengan gangguan sistem kardiovaskuler: Hipertensi
di puskesmas Sukalaksana karena hasil prevalensi yang didapat melalui
pengukuran pada umur 18 tahun sebesar 25.8 persen tertinggi di Bangka Belitung
(30.98%) diikuti Kalimantan Selatan (30.8%), Kalimantan Timur (29.6%) dan
Jawa Barat (29.4%). (Riskesdas, 2013). Menurut data yang didapatkan di
Puskesmas Sukalaksana Kota Tasikmalaya pada 3 bulan terakhir Tahun (2018),
jumlah pasien yang mengalami penyakit hipertensi masih banyak dan tidak tentu.
Hipertensi adalah peningkatan tekanan darah dari arter dan bersifat sistemik
alias berlangsung terus-menerus untuk jangka waktu lama (Lingga, 2012). Tujuan
dari karya tulis ilmiah ini adalah untuk mendeskripsikan hasil asuhan keperawatan
keluarga Tn. M dengan gangguan sistem kardiovaskuler: hipertensi. Metode yang
digunakan adalah metode deskrıftif yang berbentuk laporan studi kasus. Teknik
pengambilan data dilakukan dengan cara wawancara, pemeriksaan fisik,
pengamatan dan studi dokumentasi serta proses keperawatan meliputi pengkajian,
pelaksanaan dan evaluasi. Diagnosa keperawatan yang muncul ada 3 yaitu :
managemen regimen terapeutik keluarga tidak efektif, gangguan pola tidur dan
resiko jatuh. Asuhan keperawatan keluarga dilaksanakan pada tanggal 8-11 Mei
2019 dilakukannya implementasi berupa pendidikan kesehatan mengenai
penyakit, masalah keperawatan yang dialami Tn. M dan demonstrasi membuat
obat tradisional mentimun untuk penyakit hipertensi serta hasil yang didapatkan
bahwa masalalı dapat teratasi karena keluarga lebih bisa melaksanakan 5 tugas
keluarga mengenai penyakıt yang dialami salah satu anggota keluarganya. Dalam
pelayanan asuhan keperawatan keluarga, ditemukan adanya sedikit hambatan,
namun berkat kerjasama penulis dengan keluarga maka asuhan keperawatan
keluarga dapatdiselesaikan dengan sebaik mungkin. Asuhan keperawatan keluarga
Tn. M dengan gangguan sistem kardiovaskuler : hipertensi tentunya perlu
pemantauan kondisi kesehatan dari pihak perawat di puskesmas untuk mencegah
terjadinya masalah kesehatan yang lebih berat. Penulis

 ABSTRACT

Family Nursing care of Mr. M with the Cardiovascular System Disorders :

Hypertension in Puskesmas Sukalaksana Tasikmalaya

This paper science exposed by their attraction to the author to do the family
nursing care of Mr. M with disorders of the cardiovascular : hypertension.
Clinics in the past because of the prevalence of results obtained through
measurements on age ≥ 18 years of age amounted to 25.8 percent, highest in
Bangka Belitung (30.9%), followed by the South Kalimantan (30.8%), East
Kalimantan (29.6%) and West Java (29.4%) (Riskesdas, 2013). According to data
obtained by Puskesmas Sukalaksana of Tasikmalaya last month at 3 years (2018),
the number of patients who experienced hypertensive disease still a lot and not
necessarilv. Hypertension is the increase in blood preassure of artery that is
systemic aka sustained for a prolonged period (Lingga, 2012). The aim oj this
scientific paper is to describe the results of family nursing care of Mr. M with
disorders of the cardiovascular system : hypertension. The method used is the
method deskriftif in the form of case study reports. Engineering data retrieval is
done by means of interview, physical examination, observation and study of
documentation as well as nursing process include assessment, planning,
implementation and evaluation. Nursing diagnosis that appears there are 3
namely : therapeutic regimen management family of ineffective, disruption of
normal sleep patterns and the risk of falling. Family nursing care of implemented
on 8-11 May 2019 doing implementations in the form of health education about
thc disease, the problem of nursing experienced Mr. M and demonstrations to
make traditional medicine from cucumbers to disease of hypertension as well
as the results obtained that the issue can be resolved because more families are
able to carry out the task 5 families about the disease experienced by
one of the members of his family. In the implementation of the nursing care of
family, found the presence of a little hitch, but thanks to the cooperation of
the author with the family so the family nursing care can be completed
with as best as possible. Family nursing care of Mr. M with disorders of
the cardiovascular system: hypertension certainly need monitoring health
condition of nurses of Clinics to prevent the occurrence of health problems that
are more severe.
