Here are the requirements for the various government agencies for change of business
address:
1. Completely filled up and notarized Application Form which can be procured at the
Locational Clearance Office at the Urban Development Department, 5th floor, Old
Makati City Hall, J.P Rizal St. Brgy. Poblacion, Makati City.

Urban Development Department. You may contact her at 870- 1740.

Do not leave any blank fields, state N/A if the field is not applicable to you.

Select all appropriate sectors applicable to your organization on Pages 2 to 4 of the
form.
The Data Protection Officer and the Head of the Agency need to sign by hand.
Digital signatures will not be accepted.
Scan the notarized DPO form together with the supporting documents.
Send the scanned form and supporting documents to the National Privacy
Commission via email to: dporegistration@privacy.gov.ph.
Note: The acknowledgement email does not mean that the documents are complete,
the Compliance and Monitoring division will still validate their contents.
For complete and accurate submissions – You will receive an email acknowledging
your submission as complete. You may now request for a certificate of registration.
For incomplete or submitted documents with issues – You will receive an email with
instructions on how to complete your registration.
Regularly check your official email account and phone for our email confirmation.
1. Fill up the form which can be downloaded at the SSS Website – Employer Data
Change Request Form or as attached in this email.
2. Fill out this form in two (2) copies without erasures and alterations.
3. Indicate the Type of Employer and Type of Data Change Request by putting a
checkmark on the applicable box.
4. In filling out the portion for Tax Identification Number (TIN) in Part I- A, please
provide:
a. Business TIN for business employer
5. If requesting for change on any of your data in Part I-A, please use the new data in
filling out Part I-A of the form.
6. Submit this form to the nearest SSS branch office with the following required
documents (original and photocopy) showing the effectivity date:
a. For Change of Business Address:
i. Proof of billing or in its absence, submit any of the following:
ii. Deed of Sale of Property/Lease Contract
iii. Barangay certificate evidencing transfer to the new address
Note that all requirements are also provided for the Employer Data Change Request

1. Pass all previous requirements to register to HLURB which you have accomplished
before.
2. Submit the SEC Certificate with the new address.
3. Accomplish all forms with the new address.
