Seleksi Studi
Referensi yang diimpor ke DistillerSR adalah yang pertama
disaring berdasarkan judul dan abstrak (level 1). Pengikut
kriteria digunakan untuk skrining level 1: (1) pasien harus
memiliki POAG dan katarak dan setidaknya 1 grup harus
menerima fakoemulsifikasi sendiri sebagai pengobatan; (2) itu
studi harus dipublikasikan dalam bahasa Inggris; dan (3) menjadi an

tidak ada data tambahan yang tersedia, studi ini adalah
dikecualikan selama ekstraksi data. Perbedaan antara

Untuk hasil skala berkelanjutan seperti nilai rata-rata,
standar perbedaan rata-rata (SMD) dihitung sebagai
efek pengobatan. Menggunakan SMD alih-alih perbedaan rata-rata sebagai
memungkinkan perbandingan yang lebih mudah di semua studi. SMD dulu

DISKUSI
Tinjauan sistematis dan meta analisis dilakukan
untuk menganalisis semua data yang tersedia tentang efek phacoe-
mulsifikasi dan implantasi IOL sebagai prosedur tunggal pada
IOP pasca operasi dan penggunaan obat penurun TIO di
pasien dengan POAG. Ukuran hasil utama yang dilaporkan adalah
perubahan TIO dan perubahan antiglaucoma pasca operasi
penggunaan obat-obatan. Tiga puluh dua penelitian (1826 pasien) adalah
termasuk dalam sintesis kualitatif dan kuantitatif. Belajar
desain, populasi, lokasi, ukuran sampel, pra operasi, dan

phacoemulsification.
Significant reductions in antiglaucoma medications
were observed after phacoemulsification. Pooled effect size
estimates showed a 0.66 medications per patient reduction
in medication use 6 months postoperatively with 0.58, 0.53,
and 0.38 medications per patient reductions at post-
operative years 1, 2, and 3, respectively. The medication
lowering effects of phacoemulsification appear to diminish
lembur. In context with the IOP reductions observed,
these results further highlight the impermanent nature of
phacoemulsification's IOP lowering effects. Ekspos terhadap

outcomes in glaucoma surgical procedures. 54,66 However, if
phacoemulsification can, at least temporarily, reduce the
medication burden on patients, this may open a window
during which there is, potentially, a reduced risk of glau-
coma surgical failure. In appropriate patients, phacoe-
mulsification first, may in fact allow a physician to reeval-
uate the need for and timing of glaucoma surgery as well as
provide an opportunity to proceed surgically with a
reduced risk profile. However, the additional risks and costs
associated with a staged surgical management plan must be
carefully weighed against the risks of a combined proce-
dure. More work is necessary to accurately identify these
high responding patients preoperatively so that a proper
risk assessment can be made and ideal treatment strategy
ditentukan.
The magnitude of medication decreases after phacoe-
mulsification varied highly both within and among included
studi. Heterogeneity was also very high. Hasil ini

medication, creating variability in the dose prescribed by
doctors, and the actual dose taken or required by patients.
However, these results may also indicate variability in
individual patient IOP responses to phacoemulsification
surgery. These results give further support to the existence
of high, intermediate, and low responding populations of
glaucoma patients.

Our subgroup analyses grouped studies by baseline IOP
when examining IOP reductions as well as by baseline medi-
cation use when examining reductions in medications. Hasil
indicated that patients with higher baseline IOP showed a
greater IOP-lowering effect than patients with a lower baseline
TIO. Heterogeneity was also dramatically reduced when
studies were grouped in this manner. These results suggest that
the variability observed in IOP reductions after phacoemulsi-
fication could be explained by variability in baseline IOP.
Although this may partially explain why some patients may
benefit more from phacoemulsification, further investigation is
needed to accurately predict an individual patient's response.
The opposite trend was noted when examining
changes in medication use. Studies with higher baseline
medication use reported a smaller decrease in medications
after phacoemulsification than studies with lower baseline
medication use. Heterogeneity remained high among stud-
ies grouped in this manner, suggesting that the observed
heterogeneity cannot be explained by baseline medication
usage. Another possible explanation may be that physicians
are less willing to postoperatively reduce the medications of
patients with higher baseline medication requirements, or
those who have more advanced disease. Further studies
however are required to determine why certain patients
might experience a greater reduction in postoperative
medications after phacoemulsification surgery.
This systematic review and meta-analysis is the first to
consolidate the effects of phacoemulsification on IOP and
medication use in patients with POAG at 3 years after lens
extraction. To our knowledge it is also the first study to
include a sample size this large. However, the current study
is not without its limitations. As for any systematic review
and meta-analysis, results are only as robust as the quality
of the studies included. Downs and Black quality assess-
ment revealed sufficient quality to avoid concern; namun,
no quality scoring system is without flaw and is only as
accurate as each individual authors' methodological
reporting. Meta-analysis of observational studies is also
affected by the biases inherent in the included studies. 67
Examples of such biases that may have affected the results
of the current meta-analysis include: level of education,
ethnicity, income status, socioeconomic status, previous
ocular and nonocular surgeries, family history, other ocular
and nonocular diseases, preoperative and postoperative
medications, number of medications, comorbidities (eg,
high blood pressure, diabetes, stroke, heart conditions,
etc.), each patient's unique anatomy and genetic variation
between patients. Patient selection as well as variations in
surgical technique may also provide other sources inves-
tigator bias.
Another final consideration is that most included
studies did not report including a glaucoma medication
washout period before measuring postoperative IOP
reductions. This may bias our results as, without a washout
period, the reported IOP reductions may be partially due to
the lasting effects of patients' preoperative medication
dosis. As previous studies have demonstrated that a 3- to 4-
week washout period is sufficient to eliminate the lingering
effects of brimonidine and latanoprost, we believe that the
IOP reductions reported for follow-up periods >1 month
postphacoemulsification can be assumed to be unaffected as
the effects of patients' preoperative medication doses
should no longer be present to bias results. 68
In conclusion, this systematic review and meta-
analysis has shown that phacoemulsification as a solo
procedure does lower IOP and reduces patient dependency
on glaucoma medications. Although the clinical significance
of the observed IOP reductions do not indicate that pha-
coemulsification can replace IOP lowering surgery for all
glaucoma patients, the results of this study suggest that
certain patient populations respond to phacoemulsification
to a greater degree than others. Further work to identify
these “high-responders” is necessary, as this patient pop-
ulation may have a chance to achieve adequate IOP
reductions through phacoemulsification alone. Demikian
avoiding the need for and risks associated with additional
IOP lowering procedures. Future studies are required to
identify specific factors that are predictive of an individual
patient's IOP response to phacoemulsification. Lebih lanjut
cost-effectiveness studies as well as decision tree analyses
assessing how phacoemulsification fits within the current
glaucoma treatment paradigm are warranted to determine
if there is a cost savings associated with incorporating
phacoemulsification. Such studies as well as the current
study will be of interest to ophthalmic surgeons, patients,
governmental agencies, hospital administrators, and policy
makers to better understand and define optimal treatment
strategy for patients with this comorbidity.
