The Department of Education Regional Office 02 through the Curriculum and Learning Management Division (CLMD)
in partnership with Policy, Planning and Research Division (PPRD) shall conduct Tracer Study for the first two batches
of senior high school graduates in the region.

This tracking system aims to:
a. track the graduates of junior high school from their entry to senior high school and fulfilling the mandate to pursue
the different curriculum exits; and
b. ensure evidence based, decision making in program adjustment, interventions and initiatives.

Data to be gathered shall be kept confidential adhering to RA 10173 also known as Data Privacy Act of 2012.
Data shall only be used for SHS program review and policy recommendations.

Instructions:
Please follow these steps in accomplishing the tracking system :
1. List all names of Grade 10 completers (Batch 2016-2017) in your school.
2. Choose from the toggle list the appropriate SHS Track and Strand enrolled by the student. If Unique Track, please specify.
3. If not enrolled in your school, place in the remarks "transferred out" and specify the school where he/she trasferred. If undete
4. If students only enrolled in your school for SHS, place in the remarks "Transferred In" and specify the school where he/she g
5. Do the same thing to the other semesters.
6. Please take note to accomplish the National Certification (NC) received by the learner.
6. After Graduation, trace the exits chosen by the students from the toggle list as to Higher Education, Development of Middle-L
Employment. If enrolled in Higher Education, place the course and the school where he/she enrolled in. If Enrolled in Middle-Le
Otherwise, choose not in school, specify the reason.
7. Print the names and signatures of those who prepared and reviewed the data.
