I have understood of the activities of the Division Youth Conference to be held in Lanao del Norte National
Comprehensive High School, Baroy this July 27-28. In connection with the said event; I, as the parent/guardian:

a) I agree to my son/daughter taking part in the activities of the program and

of the registration fee of P200.
b) I confirm to the best of my knowledge that my son/ daughter does not
suffer from any medical condition.
c) I consent to my son/ daughter travelling by any form of public transport or
vehicle organised by the school to and from.
d) I agree that the school will not be held liable for any injuries/accident that
the school/faculty has no control of.
