AFFIDAVIT OF RECOGNITION AND ADMISSION OF PATERNITY

That I, JOSE RICHEL A. EULLARAN, of legal age, Filipino, and a resident of barangay
Magatas, Sibulan, Negros Oriental, under oath, do hereby depose and manifest:

That I am the Live in partner of GENEVIEVE DEL ROSARIO JAMOT which started
sometime ago and that during our cohabitation we were blessed with one child named KENT
XIANE JAMOT, who was born in August 8, 2013 at Negros Oriental Provincial Hospital in
Dumaguete City, Negros Oriental;

That by virtue of these presents, I acknowledge and recognize KENT XIANE JAMOT as
my biological son, although he used the family name of her mother which is JAMOT, as
shown in his Live Birth Certificate with an unknown father because we are not married yet;

That this affidavit is executed freely and voluntarily just to attest the truthfulness and
veracity of my statements herein.
