MOTION FOR ISSUANCE

OF WRIT OF EXECUTION
The undersigned, the wife of the late ____________, representing the
heirs of the victim ___________, unto this Honorable court, hereby moves
for the issuance of a writ of execution for this case, and hereby alleges as
follows:

That the Honorable Court, in its DECISION dated ___________, 2015,

attached and made an integral part hereof, as Annex “A”, the dispositive
portion of the CIVIL ASPECT of which states, to wit:

That the said DECISION has now become final and executory after
he lapse of more than fifteen (15) days in accordance with the
provisions of the Rules of Court of the Philippines;

That, a CERTIFICATE OF FINALITY OF ENTRY OF JUDGMENT was

issued by the Clerk of Court of this Honorable Court. Attached and made
an integral part hereof, is the copy of the said certificate as Annex “B”;

That, despite the finality of the said DECISION, the Accused did
not make good the civil aspect of the said DECISION.

PRAYER
WHEREFORE, premises considered, it is respectfully prayed that
this honorable Court will issue a WRIT OF EXECUTION as regards the
CIVIL ASPECT of the DECISION.
