Degree of severity Consequence or severity
High (3)
 People May result in long term adverse health effects, death or critical injury;
substantial loss of blood; producing unconsciousness, fracture of leg or arm, but
not a finger or toe; amputation of leg, arm, hand, foot, but not a finger or toe;
burns to a major portion of the body; loss of sight in an eye; cancer; release of
toxic substances or energy, in such a manner as to cause long term adverse
effects on the body or health.

 Property Fire, explosion, equipment or property damage greater than $250,000.00

 Production Loss of Production exceeding $250,000.00 or greater than 3 days shutdown

 Environment Fines or damage exceeding $50,000.00 for remedial work

Moderate (2)
 People May result in injury or illness with no foreseeable long term effects; release of
toxic substances in such a manner with no foreseeable long term effects, serious
injury or illness that results in lost time days; minor injuries.

 Property Fire, equipment or property damage less than $250,000.00 but greater than
$100,000.00

 Production Loss of production less than $250,000.00 but greater than $100,000.00 or less
than 3 days or greater than 2 hrs shutdown.

 Environment Fines or damage less than $50,000 but greater than $25,000.00 for remedial
work.

Low (1)
 People May result in minor injury or adverse health effects. Injury or illness that results
in fewer than three lost time days, minor cuts, bruises, temporary discomfort.

 Property Fire, equipment or property damage less than $100,000.00

 Production Loss of production less than $100,000 or less than 2 hrs shutdown

 Environment Fines or damage less than $25,000 for remedial work
