Sub: Drugs – Drugs and Cosmetics Act, 1940 and the rules made there under – Inspection at
M/s.Medplus Pharmacy, Unit of Optival Health Solutions Pvt. Ltd, Ground Floor, Plot No. 26,
Shop No.1, (West) MIG Kambar Street, Maraimalai Nagar – 603 209 – Certain Contravention
noticed – Show Cause memo issued-explanation and Particulars Submitting for-Reg.

We have received your memo dated 07.05.2019. Our director has gone out of station so
I kindly request you to grant as to give 10 days time for given the reply.

We are holding the Drug License in the above mentioned address and that outlet was inspected
by the Drug Inspector concerned on dated 31.10.2018. According to that inspection report &
Show cause notice, we are herewith giving the explanation for the violations:

1. With reference to S.No: 2 of the Show cause memo, we hereby state that at the time of the
inspection, the endorsed pharmacist we have instructed our employees to sell the medicines
only under the supervision of the endorsed pharmacist.

2. With reference to S.No. 3 of the show cause memo, we wish to bring to your kind attention
that we have maintained the prescription register and the Schedule H1 Register, due to peak
business hours, the pharmacist kept the register aside for his signature. When the inspection
took place.

3. With reference to S.No. 4 of the Show cause memo, we wish to bring to your kind notice that
the drugs shown in the table in S.No. 4 were put in separate medicines boxes since the weekly
stock audit was being done in the outlet and due to the fact that the unsold medicines and
hence there are no cash/credit memos for the unsold medicines.

Furthermore, our Company clearly mandates full compliance of all the requirements of the act
and rules there under at the time of sale of drugs to any customers. Also necessary instruction
were also passed on to the retail outlets employees to ensure strict adherence and we hereby
state that we have not violated any provision of the Drugs and Cosmetics Act and rules there
under.
