RELEASE AND QUITCLAIM

KNOW ALL MEN BY THESE PRESENTS:

I, ____________________ of legal age, Filipino, and resident of

Bayani Street, Valenzuela, after having been sworn in accordance with law,
hereby declare and acknowledge that:

1. I do hereby agree and acknowledge receipt and accept the

as financial assistance from __________________________, with principal
place of business at ______________________, as and for complete
settlement for any and all my claims for money and damages in connection
with my employment thereto.

2. By virtue of this deed, I also hereby agree and

acknowledge that _______________ has paid in full all claims and benefits
due to me, including but not limited to, salaries and other monetary benefits,
if any, as a result of my employment with _______________ and/or the
severance of said employment. I have received all my respective claims and
benefits due to me from _____________, its affiliates, partners, directors,
officers or assigns, if any, and hereby waive and relinquish all rights that I
may have to any further amounts that may be accorded as benefits. Any
consideration I received shall not be taken by me or my heirs/assigns as an
admission of liability to me of __________________ for all legal intents and
purposes.

3. I hereby further release and forever discharge, and by

these presents, do hereby release and forever discharge said
_______________, its affiliates, partners, directors, officers, and its
successors and assigns, from any claim, demand, complaint, or in all manner
of actions or claims, cause or causes of action whatsoever, and hereby
withdraw any claim or lawsuit if I have any, now or in the future, against
them under any contract, statute, or company policy whatsoever relative to
my employment with _______________ and the severance of such
employment.

4. I likewise hereby declare and acknowledge that during

my employment with _______________ I received all compensations and
benefits which are due to me under law, contract or company policy and
practice. I also admit and acknowledge that the afore-mentioned
consideration shall constitute as full, complete and final satisfaction of any
and all my claims for money or otherwise against _______________, its
affiliates, partners, directors and officers.

5. I have not been and not now suffering from any work
connected illness or disability.

6. I agree and undertake to keep all matters relative to my

employment and/or the subsequent severance thereof, in the strictest
confidence and will not be disclosed by me to any other person. I likewise
agree not to make any disparaging or false statements about
______________, its affiliates, partners, officers, directors or assigns,
whether orally or in writing.

7. I have read and willingly and voluntarily executed this

Release and Quitclaim, with full knowledge of its contents and legal effect,
as full, complete and final satisfaction of any and all my claims for money or
otherwise against _________________, its affiliates, partners, officers,
directors or assigns.
