JUDICIAL AFFIDAVIT
of witness Amy Farrah Fowler

I, Amy Farrah Fowler, after being duly sworn to in accordance with law,
hereby depose and state that I am executing this Judicial Affidavit to
substantiate the instant complaint.

PRELIMINARY STATEMENT

The person examining me is Atty. Maria Lourdes I. Cayap with office

address NO. 751 Don Efepanio Bld. 7th Floor, Payapa St. Pasay City and this
examination was conducted at her residence at Carig South, Santiago City on
January 18, 2019. I am answering the answers questions fully conscious that
I do so under oath and may face criminal liability for false testimony and
perjury.
Purpose
The testimony of the witness is being offered to prove that accused
Howard Wolowitz committed the crime charge; other matters material and
relevant to the instant case; and the foregoing Judicial Affidavit is likewise
being offered and submitted in lieu of her testimony in direct examination as
may be allowed by this Honorable Court and in compliance with A.M. No. 12-
8-8-SC.
 AND UNDER OATH, AVERS THE FOLLOWING:

1. Q : Do you swear to tell the truth and nothing but the truth?
A : I am.

2. Q : Are you aware that you may face criminal liability for false
testimony or perjury if you will not tell the truth?
A : I am.

3. Q : Please state your name, age, and address?

A : I am Amy Farrah Fowler, 28 years old, single, a resident of
Brgy. Maligaya, Santiago City.

4. Q : Why are you here now?

A : To give sworn statement by way of a judicial affidavit, the
same to constitute as my direct testimony, in the above-captioned
criminal case.
5. Q : Do you undertake to answer the questions to be asked of you,
fully conscious that you will do so under oath, and that you may face
criminal liability for false testimony?
A : Yes.

6. Q : Let us now proceed to the Complaint. Do you know the

Accused Howard Wolowitz in this case NO. 18-201 for UNJUST
VEXATION ?
A : Yes, he is the one who made the appalling act.

7. Q : Do you know the Complainant Natalie Salazar in this case?

A : Yes, she is my best friend and batch mate from Elementary up
to College.

8. Q : Why are you testifying in this case?

 A : I am testifying in this case because I personally witness the
crime done by the respondent to my freind.

9. Q : Who is with you when the incident is happened?

A : My friends, Natalie Salazar, the complainant in this case and
Bernadeth Paris.

10.Q : Who is Bernadeth Paris?

A : She is my best friend likewise of Nathalie Salazar.

11.Q : Where is this incident happed, what is the reason why you and
your best friends are there in that place and what time?
A : The incident happened at the Cafeteria of Plaza in Brgy.
Maligaya,
Santiago City. We and my two best friends were there to watch
a singing contest held at the said place at around three o’clock
in the afternoon (3:00pm) of December 1, 2019.

12.Q : Before the incident happened, what do you do with your best
friend?
A : We were watching the said competition, the complainant
whispered me in my ears that one of the guys named Howard
Wolowitz is keep staring at my friend, Nathalie Salazar.Then in a
vernacular language the accused stated that “Maganda ka sana miss
kung di lang maliit ang dibdib mo”. Out of shame, me, Natalie Salazar,
and Bernadeth Paris arose from our sits and went to the cafeteria
located near the Plaza of the said Baranggay.

13.Q : So what do you do with your friends in that Cafeteria?

A : We went there to buy our snacks and to avoid the accused.

14.Q : After then what will happen?

 A : While we are on the line waiting for a vacant table for our turn
to be served, suddenly the accused came across and hold and
furrowed the breast of our friend Natalie Salazar.We all shock
at that moment and my friend Natalie Salazar, disgracefully
cried and irritated. Then the commotion began as my friends
started to complain the shamefully acts of the
accused.Fortunately Tanods of the Baranggay came
immediately to take hold of the accused while the others
hurriedly called the Police.Then we all went to the Police station
to blotter the incident and to file our complaint against Howard
Wolowitz.

15.Q : How sure are you that the said Howard Wolowitz and the
person made a Reprehensible acts to your best friend Natalie Salazar
are the same person you are accusing right now?
A : I know him well because when we went to the Police Station for
blotter and for the filling of our complaint, there, the person
whom made a reprehensible acts confess his name to the Police
investigator.
