MEMORANDUM OF AGREEMENT

THIS IS AN AGREEMENT FOR THE PROVISION OF RADIOLOGICAL AND PATHOLOGY LAB SERVICES TO
GANGA DIAGNOSTIC & MEDICAL RESEARCH CENTRE (P.) Ltd. AND PRANAAM ENT AND EYE CARE

This agreement made at Raipur Dt - 10-05-2019

And

GANGA DIAGNOSTIC & MEDICAL RESEARCH CENTRE (P.) Ltd.

Opp MMI Hospital, Lalpur, Pachpedi Naka, Raipur (C.G.)
 AGREEMENT FOR PROVSION OF RADIOLOGICAL AND PATHOLGY LAB SERVICES BETWEEN
GANGA DIAGNOSTIC & MEDICAL RESEARCH CENTRE (P.) Ltd. AND PRANAAM ENT AND EYE CARE

1. Duration

From 10-05-2019 until terminated by either party (agreement to be reviewed annually)

2. Service objectives

The objective of this Agreement is to secure for RADIOLGY AND PATHOLOGY LAB SERVICES to a
level satisfies the requirements of the Aayushmaan Bharat Yojna.

Service to be coverd

The main elements of the service to be covered by this agreement are

 Provision of Radiology services like CT Scan and XRay.

 Provision of pathology Lab services like Blood Sample analysis and Histopathology.

3. Transport and Collection of Samples

GANGA DIAGNOSTIC & MEDICAL RESEARCH CENTRE (P.) Ltd. will make arrangements for the
transport services like ambulance for the patients to and from the PRANAAM ENT AND EYE
CARE.
GANGA DIAGNOSTIC & MEDICAL RESEARCH CENTRE (P.) Ltd.will make arrangements for the
collection of Blood and Tissue Samples and also the reports for the same To and From PRANAAM ENT
AND EYE CARE in appropriate time period.

4. Charging

PRANAAM ENT AND EYE CARE will be responsible for paying for the abovementioned services
to GANGA DIAGNOSTIC & MEDICAL RESEARCH CENTRE (P.) Ltd. on behalf of the patients
covered under Aayushmaan Bharat Yojna under treatment in PRANAAM ENT AND EYE CARE and
that no charges will be collected from these pts.
However the Patients not covered under Aayushmaan Bharat Yojna will pay by themselves
directly to the GANGA DIAGNOSTIC & MEDICAL RESEARCH CENTRE (P.) Ltd. as per the rates
decided by its Management.
 These are charges agreed for the aforesaid services rendered to Pts under Aayushmaan Bharat
Yojna.

1. CT PNS – 1750/-
2. CT TEMPORAL BONE – 2750/-
3. CBC – 250/-
4. Pt/INR – 175/-
5. HIV – 240/-
6. HBsAg – 250/-
7. RBS – 40/-

5. Compliance

GANGA DIAGNOSTIC & MEDICAL RESEARCH CENTRE (P.) Ltd .will wish to assure that reporting
will be done by Qualified Radiologist and Pathologist and appropriate quality control measures
are followed as per the latest guidelines.
