1. Identifikasi gelang yang terpasang dengan benar dan pastikan diperiksa 2x

2. Formulis pesetujuan ditandatangani
3. Lokasi operasi ditandai oleh ahli bedah
4. Periksa kategori informasi gryang masuk pada status pasien dengan benar,
kategori informasi grafik obat, kategori informasi grafik resep, catatan, kategori
informasi grafik cairan
5. Sinar X yang disertai informasi grafik pada catatan pasien
6. Jika diketahu terdapat alergi ( pasang gelang alergi berwarna merah dan tandai
Ya/Tidak)
7. Pasangkan dan tutupi dengan tutup kepala. Kenali posisinya
8. Melepaskan gigi palsu ( jika tidak maka dilepas dibangsal terlebih dahulu, tolong
diletakkan)
9. Lokasi pada bagian yang hendak dioperasi dicukur
10. Hapus/bersihkan kutek pada kuku
11. Lepaskan semua perhiasan
12. Periksa penggunaan tindik agar dilepaskan
13. Pakaian yang sudah using / lapisan ketat / celana tanggung ( short CD )
14. Sebelum medikasi diberikan harus sesuai dengan grafik informasi anastesi

Patient coming to Alexandra Hospital and his name is Gary Stephens. He born at
5/10/1974. An he is the war in Alexandra hospital number 12B, the name of his
consultant is H.Perowne. This is the wound assessment form:
Mr.Gary Stephens coming to Alexandra hospital at 8/2/2008. He have problems
woud site at the L calf. The wound description L calf granulated, frequency of
dressing bd, he has not antibiotics, his surrounding skin condition healthy, he has not
exudate, the exudate type N/A, he has odour present, and he has not debridement. He
has dressing products and this name non-adhesive dressing (NAD). His wound
closure sutures, and comments of L calf is wound for review by Vascular Team on
Mon.

has antibiotics, his surrounding skin conditions dry, he has small amt exudate, the
exudate type serous, he has odour present, and he has surgical debridement. He has
dressing products and this name non-adhesive dressing (NAD). His wound closure
clips, and comments of L calf is for review by Vascular Team on Mon.

Hospital. He born at 5/10/1974. An he is the war in Alexandra hospital number 12B,
the name of his consultant is H.Perowne. This is the wound assessment form of Mr.
Gary Stephens :
Mr.Gary Stephens coming to Alexandra hospital because he have problems woud
site at the L calf. The wound description L calf necrotic, frequency of dressing is
daily, he has oral antibiotics, his surrounding skin conditions is macerated, he has
moderate exudate, the exudate type is haemoserous, he has not odour present, and he
has mechanical (wet to dry dressings) debridement. He has dressing products and this
name antimicrobial. His wound closure clips, and comments of L calf is wound
intact-nex dressing in two days.

site at the L calf. The wound description L calf infected, frequency of dressing is 3 rd
daily, he has IV antibiotics, his surrounding skin conditions is inflamed, he has heavy
exudate, the exudate type is purulent, he has not odour present, and he chemical
debridement. He has dressing products of hydrating . His wound closure open wound,
and comments of L calf is wound intact-nex dressing in two days.
