KNOW ALL MEN BY THESE PRESENTS:

This Deed of Consolidation Subdivision with Waiver and Partition

pursuant to the CONSOLIDATION SUBDIVISION PLAN

XXXX , widow, of legal age, Filipino citizen with residence and postal

W I T N E S S E T H:

Whereas the parties are the registered owners of two adjacent parcel of land

Whereas XXXXXXXXXXXX is the registered owner of that parcel of land

covered by _____________, more particularly described as follows:

Whereas __________________ is the true and registered owner of a parcel

of land covered by ______________, more particularly described as follows:

Whereas both parties manifest their conformity to the Consolidation

Subdivision Plan of the aforementioned Lots with a total area of

Whereas, pursuant to the Consolidation Subdivision plan of Lots ____ and

________________ approved on____________________, the consolidated lots
were subdivided into three (3) lots, to wit:

Whereas, pursuant to the said Consolidation Subdivision plan, LOT 1 and

properties sufficient to support her and her family in a manner appropriate to their
needs;

Whereas, due to the demise of XXXXX, the spouse of XXXXX; his estate
was settled by virtue of an EXTRAJUDICIAL SETTLEMENT OF ESTATE
AMONG LEGAL HEIRS WITH SIMULTANEOUS WAIVER OF
RIGHTS AND SALE on _____ and registered as Doc. No. ___; Page no.: ___,
Book no.: ___; Series of _____ by the undersigned Notary Public. The Estate
includes the parcels of land identified as ________________of the said
consolidation subdivision plan.

Whereas, the heirs of XXXX have sold, conveyed and transferred in a

manner that is absolute and irrevocable that parcel of land identified as LOT 1 in
the CONSOLIDATION SUBDIVISION PLAN to XXXXX;

Whereas the other heirs of ______ namely

____________________________ have waived, quitclaimed and transferred that
parcel of land identified as LOT 2 in favor of their mother/co-heir xxxxx;

Now therefore, for and in consideration of the above agreement, the

PARTIES herein prays that the KATIBAYAN NG ORIGINAL NA TITULO
Blg xxxx and KATIBAYAN NG ORIGINAL NA TITULO xxxx be
CANCELLED by the Register of Deeds of xxxx, and in lieu thereof to
ISSUE THREE (3) NEW CERTIFICATES OF TITLE in favor of xxxx for
LOT 1, xxxx for LOT 2 and xxxxx for LOT 3;

This refers to a CONSOLIDATION SUBDIVISION WITH WAIVER

PARTITION which consist of five (5) pages, including this page in which the
acknowledgement is written, all pages duly signed by the parties and their
