I, JUAN DELA CRUZ, of legal age, Filipino, married and a resident of 1234
General Luna St., Bangkal, Makati, after having been duly sworn to in accordance
with law, hereby depose and say that:

1. I am the President of Bank of the Philippine Islands;

2. I represent the plaintiff corporation in the above-entitled complaint;

3. I have caused the preparation and filing of my complaint after reading

the contents thereof which are all true and correct of my personal knowledge and
based on authentic documents;

4. I further, certify that I have not filed a similar action involving the same
before the Supreme Court, Court of Appeals or ant agency or tribunal, that to the
best of my knowledge there is no pending case involving the same before the
Supreme Court, Court of Appeals or any agency or tribunal; and that should I came
to know later that a similar action is pending before the Supreme Court, Court of
Appeals or any agency or tribunal, I will undertake to report that fact within five (5)
days therefrom to the Court wherein this pleading is filed.
