RELEASE AND QUITCLAIM

KNOW ALL MEN BY THESE PRESENTS:

COMPANY]   in full and complete payment and satisfaction of my salaries and of whatever

under the laws, existing agreements and/or company policy or practice, as well as any all claims
of whatever kind and nature such as but not limited to claim or claims for any/all claims by way
of underpayment of salary/wages, non­payment of overtime pay, holiday pay, holiday premium,
rest day premium, service incentive leave, 13th month pay, non­payment of ECOLA, separation
pay,   unpaid   night   differential,   which   I   have   or   may   have   against    [NAME   OF   THE
COMPANY] arising from my former employment with the said company.

WHEREAS,  my   probationary   employment   with   the   company   ended   on

WHEREAS, I do not have any further claims or causes of action against  [NAME OF
THE COMPANY] or any of its directors, stockholders, or officers.

NOW, THEREFORE, for and in consideration of the foregoing premises I have remised,
released   and   forever   discharged   and   by   these   presents   do   for   myself,   my   heirs,   successors,
executors   and   administrators,   remise,   release   and   forever   discharge  [NAME   OF   THE
COMPANY],   its   successors   and   assigns   and   all   its   officers,   directors,   stockholders   or
employees, of and from any and all manner of actions, claims or demands whatsoever against
[NAME OF THE COMPANY]  which I have or ever had by reason of, in connection with or
arising out of my employment with the said [NAME OF THE COMPANY]  I hereby undertake
not to sign, execute, and make any and all affidavit, statements, letters, etc. that is against the
company, in furtherance of any claims, cause or case of any and all employees of [NAME OF
THE COMPANY], whether past or present.
