De Lemos and a resident of Valenzuela, Metro, Manila under oath depose and state:

1. That I was married to Elmer De Lemos on _________________ at

2. That Elmer De Lemos is the registered owner of a parcel of land covered and
embraced by Transfer Certificate of Title No. 039-2013004926 situated in
Barihan, Malolos City, Bulacan with an area of 600 square meters.

3. That we recently discovered insofar that my name in the above transfer

certificate of title was erroneously entered as CRISTY Z. DE LEMOS in lieu of
my correct name MA. CHRISTINA Z. DE LEMOS.

4. That I hereby certify that my true and correct name is MA. CHRISTINA Z. DE
LEMOS as indicated in my Certificate of Marriage which is hereto attached for
your easy reference.

5. That the said affidavit is executed to correct my name in the said Transfer
Certificate of Title No. 039-2013004926 for all legal intents and purpose.

2013004926 situated in Barihan, Malolos City, Bulacan;

3. We hereby certify that CRISTY Z. DE LEMOS and MA. CHRISTINA Z. DE

LEMOS refers to one and the same person.

4. We are not related by affinity or consanguinity to the above mentioned

person; and
5. We are executing this affidavit to confirm and affirm the foregoing facts and for
whatever legal purpose it may serve.
