Sub.: Appointment for the Position of X-Ray Technician

Dear Mr………………………. ,

With reference to your application and the subsequent interview with us, we are pleased to appoint you
as

X-Ray Technician in our X-Ray Department at our Tripathi Hospital, Nodia on the

following terms and conditions:-

1. Date of Appointment

Your appointment is effective from ……………….

2. Compensation Package

You will be paid Rs. 15,200/- CTC per month.

3. Probation & Confirmation

You shall be on probation for a period of Six Months during which time your performance and conduct

will be assessed to ascertain if you have come up to the expectations of the Management. In case you
do

not receive a letter of confirmation at the end of the aforesaid period it will be considered that your

probation period has been extended. The extension of your probation period or confirmation will be

conveyed to you in writing.

4. Duties & Responsibilities :


a) You will be responsible for sincere, efficient and satisfactory discharge of duties assigned to you from

time to time and shall devote sufficient time and attention to the business of the Hospital.

b) During the terms of your employment with the management, you shall not engage yourself in any

other business or occupation with or without remuneration except with the written permission of the

Management.

c) You are liable to be called for any urgent emergency duty where your presence is considered essential

even outside your working hours.

d) You will be true and faithful to the management in all accounts, dealings and transactions whatsoever

in relation to the business of the hospital and shall, at all times when required, render a true & just

account thereof to the management or to such person(s) as authorized by the management for the

purpose.

e) You will have to perform as per the job description given to you up to the satisfaction of the Hospital

Authorities & the Management.

f) You will not be allowed any kind of practice/ consultation at home or elsewhere in your own name or

any relative or anybody else in every form whatsoever.

5. Secrecy :

During your employment with our establishment (except as far may be proper in the normal course of

business & for the interest of our establishment) or at any time thereafter you shall not divulge to
any

unauthorized person any information or documents of a confidential nature, any of its secret processes,

any sketch, drawing, specification, data or any other material which may be in your knowledge or

possession relating to the Hospital’s activities.

6. Physical & Mental Fitness:

Your appointment has been made subject to your being found medically fit by a Doctor of the Hospital’s
choice. At any time in the future if it is found that you are not physically & mentally fit to discharge
your

duties diligently & efficiently on continuous basis, the Management reserves the

right to have you medically examined by a Doctor of its choice and in case you are not found mentally or

physically fit, your services are liable to be terminated. The decision taken by the management in this

regard, taking into consideration the medical report, would be final & binding on you.

7. Posting & Transfer :

a) You shall be posted at the Tripathi Hospital, Nodia.

b) You may be required by the Hospital authorities at its sole discretion to work in shifts including night

shift.

c) You are also liable to be transferred from one location/Unit to another Hospital/establishment owned

by the management during the course of your employment. You may also be transferred to a location

or Unit which is not there at present but will come up in future.

d) Your services can also be transferred, seconded or loaned to any other associate / sister concern in

existence or to be formed in future during the course of your employment.

e) In case of transfer, you will be covered by the rules and regulations of the Unit /Company to which

you are transferred.

8. Service Rules

a) You will be entitled to leave as per rules of the Company applicable to your category of employees

from time to time.

b) You will be also entitled to be covered as per the provision of E.S.I Act 1948 & EPF 1952, if

applicable.

c) Your address indicated above shall be deemed to be the correct address for sending any

communication to you unless changed in writing. It shall, however, be your duty to intimate any

change in your address in writing within seven (7) days of its occurrence and get it recorded in your

personal file and other Hospital records.


d) You will be liable for retirement from the Hospital on completion of 58 years of age.

e) On cessation of your employment, you will return to the management all papers and documents

relating to the business of the Hospital which are at that time in your possession and you will not

retain any copies or extracts there from.

f) You have to punch your attendance by giving your thumb impression daily & also mark your

attendance in the register placed in your department .In case you leave your working place you have

to sign on the movement register placed in your department & also punch your attendance in

punching machine.

9. Termination of Service :

a) During the period of probation, initially fixed or subsequently extended, your services may be

terminated without any notice from either side without assigning any reason thereof.

b) After confirmation, this appointment may be terminated by either side without assigning any reason

by giving one months notice or payment in lieu thereof.

c) In case of any misconduct your services are liable to be terminated by the Management without any

notice or payment in lieu thereof.

d) Your services are liable to be terminated in case any particulars furnished by you to the Company are

found incorrect or your ex-employers / Police Authority have furnished adverse report regarding your

conduct / character or you have been convicted in any criminal proceedings.

e) In case you are found absent for eight (8) days without prior permission of the Management in writing

or you proceed on leave without prior sanction or overstay the sanctioned leave without any approval,

it will be considered that you have voluntarily abandoned the employment on your own accord and

your services shall automatically come to an end.

In all other matters not specifically mentioned herein, you will abide by the service rules / standing
instructions as applicable, enforced, amended or altered from time to time during the course of your

employment
