4th team meeting Identifikasi penyebab modus kegagalan, tugaskan anggota tim untuk
berdiskusi dengan petugas pada unit yang terlibat dalam proses untuk
memperoleh masukan tambahan (langkah 3)
5th team meeting Tuangkan modus kegagalan dan penyebabnya pada lembar kerja HFMEA
(langkah 3). Lakukan analisa hazard (langkah 4) Identifikasi tindakan
