KATA PENGANTAR

Dengan memanjatkan Puji syukur kehadirat Allah SWT, karena atas berkah

rahmat, serta hidayah-Nya penulis dapat menyelesaikan tugas akhir I ini dengan judul:

“Sistem Informasi Layanan Pasien Berobat Puskesmas Kebonagung”.

Dalam penyusunan tugas akhir I ini penulis tidak terlepas dari bantuan,

dukungan dan dorongan dari berbagai semua pihak, baik secara langsung maupun

tidak langsung. Untuk itu pada kesempatan ini dengan segala kerendahan hati penulis

ingin mengucapkan rasa hormat dan terima kasih yang sebesar-besarnya kepada semua

pihak yang telah membantu dan meluangkan waktunya dalam menyelesaikan tugas

akhir I ini.

Penulis menyadari tugas akhir I ini masih banyak kekurangannya dan jauh dari
