VISION, MISSION, GOALS AND OBJECTIVES (VMGO)
I. VISION-MISSION STATEMENT OF MY MESSIAH SCHOOL OF CAVITE

THE VISION
The school envisions to empower students to acquire, demonstrate, articulate and value
knowledge and skills that will support them, as life long learners, to participate in and contribute
to the global world and practice the core values of the school:
 Learning through innovation
 Finding the courage to challenge the convention
 Pursuing improvements
 Working to continually exceed expectations
 Growing by learning
 Striving to develop potentials
 Making an active participation to local and global community
THE MISSION
The school missions to empower all students to apply their acquired skills and
knowledge, and to rely upon their personal attributes to lead productive lives and become
contributing members of the global community.
THE OBJECTIVES
MY MESSIAH SCHOOL OF CAVITE, aims to:
 Provide the necessary training in the basic learning subjects geared towards the
development of the learner’s potentials, talents and abilities according to his needs and own
rate of learning which will enable him to educate himself for the rest of his life.
 Facilitate the proper development of a child’s aptitude through a comfortable and
systematic learning process concretized with work and play.
 Provide an environment that will help the pupil to become a well-balanced citizen who are
prepared to take their proper places as individuals and as a member of their respective
social groups or community in a democratic society
 Teach basic health habits, the formation of desirable habits that will make each learner
physically, mentally and emotionally healthy equip the individual learner with social,
spiritual and moral values which will enable him to think and live in a manner befitting an
educated Christian Filipino in the society
Give the necessary love, care, understanding and patience to the pupils for them to feel
and eventually can share the same love, understanding, care and dedication to others.

II. GUIDANCE SERVICE OFFICE’S VMGO

VISION

The Guidance Services Office aims to develop well-adjusted and globally competitive individuals
equipped with values that will help them assess their purpose in life to create a satisfying relationship
amongst their families, peers, and community.

MISSION

To realize this vision, we commit ourselves to provide assistance in a holistic attainment and
development of necessary proficiencies and life skills needed by the students through quality
program and services.

GOAL

 To offer a quality program that provides students with various activities to help them boost
their self-esteem and be more self-aware to discover full potential, improve relationships to
effectively relate with other people and unveil a deeper meaning and purpose in life.

OBJECTIVES
The Guidance Services Office aims to facilitate students:
a.) Recognize interests and goals through self-discovery and be able to identify strengths and
weaknesses to reach full potential.
b.) Make wise and informed decisions to independently managed their own lives.
c.) Develop positive values and attitudes to effectively relate to different individuals which will
help improve relationships.
d.) Enhance academically by way of boosting self-esteem and attain high level of professionalism
to improve quality of life through career/academic/vocational counseling, follow-up and
monitoring services.
e.) Obtain life skills that are essential towards holistic development by means of individual/group
counseling and guidance seminars and enhancement programs.
f.) Appraise various personalities and capacities through psychological assessment.

OUR SERVICES

This chapter presents the different guidance services given to MMSC community to have a
comprehensive detail of what the Guidance Office is all about.

GUIDANCE & COUNSELING AS GOD CENTERED

Guidance is an integral part of the educational program of My Messiah School of Cavite,


which seeks to provide, as needed, opportunities to all students to help them grow intellectually,
spiritually, socially, emotionally and physically. It is not only developmental by design but it would
also address their need for a personal relationship with God, knowledge of self and others, needs
for competence as well as educational excellence who in turn will become contributors toward
nation-building.

It is a ministry, which is concerned for the optimum growth of every student and the
realization of his/her potentialities for individual, social and spiritual needs. Likewise, it is
convinced that a meaningful life can only be experienced in its fullest on the basis of a true
relationship with God.

It is a ministry that recognizes the right of every student to personal assistance, values the
worth of persons and is inspired by God. Likewise, it ensures confidentiality, love, authenticity,
respect and acceptance of its counselees.
GUIDANCE & COUNSELING AS STUDENT-FOCUSED
Since it is an integral part of education, spiritual and personal formation, it is concerned with
a comprehensive study of the student in his/her cultural/ ethnic affiliations, circumstances, and
family of origin, which may provide sufficient understanding of his/her self. Appropriate
counselling techniques will be utilized and scientific measurement tests will be used, as they are
available.

It is focused on the student’s academic successes and skills, developmental needs, and also
in his/her adjustments in the critical situations of life. Thus, it seeks a program that will assist
them toward realizing and actualizing their best self.
GUIDANCE & COUNSELING AS FUNCTIONAL CENTER
Teachers, parents, and school administrators’ cooperation is very much encouraged in the
carrying out and in the implementation of the objectives, programs and activities of the center.

The task is entrusted to those who have the necessary training and experienced. The guidance
programs will be under constant evaluation in terms of its effectiveness.

It is responsive to the student’s needs.


1. The Guidance Program addresses the developmental adjustment needs of the students.
2. The Program considers the unique culture and climate of the specific population and
environment it serves.
3. Program offerings are based on regular, systematic assessment of the students needs and
environment.
4. Guidance is a continuous process throughout the person’s stay in school.
5. Guidance activities are specifically planned and developed to ensure Program
effectiveness.
6. The Guidance team consist of school counselor, social workers, teachers, administrators
and other educational professionals and staff who work together for the welfare of the
students.
7. The Guidance Program recognizes the need to provide objective evidence of
accomplishments and the rationale for its continued existence.

It recognizes the student as an individual and premised on the following:


1. Every individual has needs that must be attended to.
2. Guidance should be extended to all.
3. Every individual is unique and possesses worth and dignity.
4. Human beings can develop their own insights.
5. All individuals have the right and capability to make decisions and plans.

The program in the hands of qualified personnel.


1. The school counselor is a role model of positive human relations.
2. Professionally trained guidance personnel are indispensable for ensuring competence,
leadership, and direction.
MMSC Guidance Services Office shall be regulated by the policies and procedures contained
on this Manual and by the over-all policies of the school. The office is under the Principal.
Office hours start at 8:00 in the morning and ends at 5:00 in the afternoon from Monday to
Friday.

Below are the different services offered by the Guidance Office:


1. ADMISSION & INDIVIDUAL INVENTORY SERVICE
All incoming new students and transferees undergo admission test prior to
enrolling in My Messiah School of Cavite for academic placement. The Center
works with the Admission Committee in carrying out the admission policies for
proper entrance of applicants who has the capacity to meet the standards of My
Messiah School of Cavite.
It systematically collects, evaluates, and interprets data to identify the
characteristics and potential of every client (Villar 2009). The information usually
stored in an individual folder where the data accumulated about each student are
kept while the student is a bona fide recipient of training and services of the
institution.
2. INFORMATION SERVICE
It consists of gathering and spreading of information about the different guidance
activities, vocational opportunities, and educational information for better
adjustment and personal growth.
The three major types of information that can be disseminated to students
(Villar 2009):
A. Personal-Social. This has to do with how one can improve oneself or
how one can improve one’s relationship with others. This could be in a
form of group guidance, workshops, films, lectures, seminars, and
panel discussions.
B. Academic-Educational. This information concerns the educational
setting that one is in or might consider. This could be done through
brochures/bulletins/websites/orientations/career guidance and
campaign.
C. Vocational-Occupational. This covers vocational-occupational
opportunities the students can pursue in college. This may take in the
form of group guidance, career weeks and orientation days, posters,
seminars, symposia, brochures, bulletins, books and films.
3. TESTING/APPRAISAL SERVICE
This service consists of the administration, analysis and interpretation of
standardized psychological test or questionnaires. The Center provides paper-based
standardized test and psychological assessments to appraise students’ abilities, aptitudes,
scholastic achievement, interests, personality and attitudes. Information acquired from
these tests data are used to enhance their self-understanding which are needed in
forming realistic improvements, adjustments, and realization of the student’s potentials.
4. COUNSELING SERVICE
Counseling is a one-to-one helping relationship that focuses on a person’s growth
and adjustment and problem solving and decision-making needs (Gibson &

Mitchell,2008). It is the counseling service that integrates all the data gathered
about the individual and his/her environment, in order for them to make sense
(Villar, 2009).
Types of Counseling According to Areas Covered
Below are the general types of counseling taken from the Dr. Villar’s book on
Implementing a Comprehensive Guidance and Counseling Program in the Philippines.
1. Academic/Educational. It focuses mainly on school or academic concerns: school
selection, school entry, school adjustment, and school maintenance. This also focuses on
college plans, which may involve shifting to another course.
2. Vocational/Occupational/Career. It focuses mainly on the selection of or the proper
preparation for the world of work. Decisions on whether to pursue a degree or non-
degree course, field of concentration for college and/or after college, and the appropriate
school for a selected program of study are usually the main targets especially in the latter
part of high school, since they relate to the kind of work or career that is being prepare
for.
3. Personal/Social. This type of counseling can be extended to anyone at any age and usually
focuses on the interpersonal and intrapersonal growth of a person. It can be preventive,
developmental, or remedial.
a. Diet Counseling – considered the work of the dietitian or the nutritionist,
school and psychological counselors may find themselves doing some of it.
Referral are best, but some people need initial counseling to encourage them
to seek help.
b. Crisis Counseling – is a helping process to assist an individual or group to
survive an unsettling event so that probability of debilitating effects (e.g
emotional trauma, posttraumatic loss reactions or physical harm) is
minimized, and the probability of growth (e.g new coping skills, new
perspective on life, or more options in living) is maximized.
c. Grief or Bereavement Counseling – it can be helpful to normally functioning
individuals who have suffered an overwhelming loss.
d. Pastoral Counseling – this type of counseling is usually conducted among those
with moral-spiritual issues. The training involves theology and behavioral
science.
e. Leisure Counseling - this is more on educating and counseling people on how
to make the best use of their free time which will have great impact on the
physical and mental well-being of individuals and society in general.
f. Addiction Counseling: Substance Abuse, Cyber Addiction and Addiction to the
Addicted.
Types of Counseling According to Participants

1. Individual Counseling – This process is initiated when a state of psychological contact


or relationship is established between the counselor and the client; it progresses as
certain conditions essential to the success of the counseling process prevail.
2. Group Counseling – It is an activity designed to provide individual with information or
experience that promote their career or educational understandings and personal-
social growth and adjustment.
3. Multiple Counseling – It is a process wherein more than one counselor handles an
individual, a couple or a group.
Counseling Approaches
Below are the general types of counseling approaches the Guidance Counselor
may apply or adapt according to the client’s need:
1. Client-Centered Counseling/Person-Centered Approach
The goal is to help the client arrive at self-actualization.
2. Eclectic Counseling
The approach allows the counselor to use a combination of the different
counseling techniques.
3. Behavior Modification
This approach requires that the counselor be more directive as the counselor gives
prescriptions and instructions.
4. Reality Therapy
Work is on the here-and-now behavior rather than on the past or on feelings since
the latter could be used as excuses for irresponsibility.
5. Rational-Emotive Therapy
Therapy should focus on the development of rational ideas or changed philosophy
of life which all human beings are capable of.
6. Gestalt Counseling
This approach aims to help the individual develop self-integration.
7. Transactional Analysis
The client is assisted in gaining social control of his/her life by learning to use all
ego states where appropriate.
8. Brief Therapy
The counselor is directive in that a client or family will be instructed on how to
specifically behave in particular situations or asked to try out new behaviors or
new ways of thinking about the situation.
9. Solution-Focused Therapy
It is guided by three ground rules: if it is not broken, don’t fix it; if it works, do
more of it; if it does not work, do something different (Villar, 2009)
10. Cognitive Therapy
The goal is to alter negative thoughts that create negative emotion and
consequently felt problems.

5 REFERRAL SERVICE
“Referral is usually understood as the action taken by persons within the
institution who see that a particular person needs counselor assistance. In a school
setting, teachers, administrators, parents, and other students may refer a specific
student for counseling. This is an accepted practice, an it presumes that the
counselor is seen as able and available, and that counseling services are well-
publicized within the school community.
6 FOLLOW-UP
We will follow up the whereabouts of graduates in order to secure relevant
information needed by My Messiah School of Cavite to diagnose and evaluate the
strengths and weaknesses of its curricular program for the benefit of current students.
The service will be linked to the services of the Alumni Office and Records Center.
7 SPIRITUAL AND PERSONAL FORMATION
It involves values formations which focus on the foundations for the Christian faith,
values, character building, ethics and others.

It also includes information concerning “School Success Skills, Assertiveness Training/


Conflict Resolution Skills, Social Skills and Building Healthy Self-concept.” Lessons on
these are taken during Classroom Guidance.
8 PEER OUTREACH PROGRAM
A process in which counselor trains students as peer mediators, conflict managers,
tutors and mentors to their fellow students. In other words, it is a service wherein
students from different levels who are interested to join the organization will be trained
and become Peer facilitators. It is their tasks to become referral agents, assist the center
with the activities, and inform other students with the different guidance services.

POLICIES AND PROCEDURES


This portion discusses the policies and procedure of each program and services of the Guidance
Services Office.

1. INDIVIDUAL PLANNING SERVICE


A. INDIVIDUAL INVENTORY
1. Creation of Student’s Individual File
1.1 Provisions for Newly Enrolled Students
This applies to all new and transferee students to be accomplished during the
month of July.
a) Visit the guidance office to get the Individual Inventory Form or check out
(google drive link here) for the online Individual Information Form (MMSC
GSO-0001, MMSC GSO-0002/2A; please refer to Appendices A-A.1)
b) Type your ID number/ student’s email address (Note: Refer the ID number
from the School ID)
c) Fill out all the necessary data honestly.
d) Click Submit button to save the information stated.
e) The Counselor shall print the finished output and place it in their
individual folder.
1.2 Provision for Incoming Senior and Junior High School Students
a) Visit the guidance office to get the Individual Inventory Form or check out
(google drive link here) for the online Individual Information Form (MMSC
GSO-0001, MMSC GSO-0002/2A; please refer to Appendices A-A.1)
b) Save and name the file with your full name, send it to the Guidance FB
page named MMSC Guidance Office for online registration.
c) Those who’ve chosen to answer the hard copy of the said form may
submit it personally at the Guidance Office.
d) The Counselor shall print and file the finished output and put it in their
individual folder.
1.3 Provisions for ALL STUDENTS
a) All students shall have a Student’s Services Record Card kept by the
counselor.
b) The Counselor shall record the guidance services/activities participated
by each student through the card.
c) Each time the student came to the office, the counselor shall pull out the
card to record the time and purpose.
d) This may help keep track of the students who actively availed the
programs and services conducted.
2. File Update

2.1 Provisions for Returnee Students


a) Visit the Guidance Services Office and consult the counselor during
enrollment.
b) The counselor may then retrieve the file.
c) Interview with the counselor shall be done with the retrieved file for
updates especially with the contact numbers and addresses.
d) If the student has not been with school for the last 1-2 years, the student
will have to accomplish all the requirements again.
2.2 Provisions for Completers /Graduating Students
a) Pull-out the file of students who graduated.
b) Put them in one folder with the label of the school year finished.
c) Separate the folder and put it on the inactive files.
2.3 Provision for Students who Filed Leave of Absence or School Leaver.
a) Pull-out the file of students who filed for a leave of absence or want to
transfer to another school.
b) Separate the folder and put it on a separate folder labeled students who
filed leave of absence.
3. Record Filing
Compile and record students’ activity sheets, responses to
questionnaires, call slips, interview reports, appointment, monitoring
forms and any other relevant forms and to be placed in their individual
folders.

B. Client Encounter
It is a way of the Guidance Counselor to better assist student’s concerns through
interviews.
1. Initial Interview
1.1 Provisions for Newly Enrolled Students
a) The student may first fill-out the appointment form and register on
the visitor’s logbook.
b) The counselor shall conduct an initial interview for 5-30 minutes.
c) The counselor may discuss with the student the schedule for the next
possible meeting if it is necessary.
d) The student may evaluate the office using the Feedback form.
e) The counselor shall summarize the interview at the Counselor’s
Interview Report (MMSC GSO-0004; please refer to Appendix E)
2. Interview with Shiftees (SHS)
The student must completely discuss the decision to shift with the
counselor to clear the vague details of the course and to align his/her
interests and abilities as noted on his individual file.
3. Exit Interview

3.1 Provision for Completers/ Graduating Students


a) The student may first fill-out the appointment form and register on
the visitor’s logbook.
b) The counselor shall retrieve the accomplished print-out forms
(MMSC GSO-0007: Exit Interview Form; please refer to Appendix I) of
the student.
c) Students must form a group of 6-10 members for the interview to be
conducted at the Guidance Office.
d) Discussions and clarifications of the answers shall be facilitated by
the counselor for 20-45 minutes.
e) The student may evaluate the office using the feedback form.
3.2 Provision for School Leavers
a) After getting the Clearance for Request of School Documents at the
Office of the Registrar and proceed to the offices involved for the
clearance; you may directly go to the Guidance Services Office.
b) The student may first fill-out the appointment form and register on
the visitor’s logbook.
c) Ask for the MMSC GSO-0009: School Leaver’s Checklist and fill-out
the form.
d) Give the accomplished form along with the clearance form to the
counselor.
e) The counselor may conduct an interview for 5-15 minutes to update
the file of student.
f) The student may evaluate the office using the MMSC GSO-0015:
Feedback Form; please refer to Appendix R.
4. Routine Interview
4.1 Provisions for New Students
a) Interview shall be done on the first two months of the school year.
b) The student may first fill-out the appointment form and register on
the visitor’s logbook.
c) Interview shall last 5-30 minutes depend on the situation.
4.2 Provisions for Continuing Students
a) Interview shall be done on the first two months of the school year.
b) The student may first fill-out the appointment form and register on
the visitor’s logbook.
c) Interview shall last 5-30 minutes depending on the situation.

C. Testing Service

This service is handled by a Psychometrician/Testing Officer/Guidance


Counselor. He/she is responsible in administering, scoring, interpreting
psychological test results given to the students.
1. Test Takers/Examinees
1.1 Provisions for Test Takers/Examinees
a) Observe proper conduct before/during and after the exam.
b) Before the testing begins, the Testing Officer shall introduce
himself/herself to the examinees and explain the purpose of the
exam.
c) Examinees have to silent or turn-off their gadgets that might distract
during testing and observe way that could keep distractions to a
minimum.
d) Only the necessary materials should be on the working table of the
examinees. Pencils shall be provided.
e) Listen to the instructions carefully.
f) Examinees are encouraged to raise questions and concerns before
the testing starts. She/He may notify the testing officer of these
concerns.
g) Do not open the test booklet once receive unless told to do so.
h) Solving or writing solutions on the test booklet is prohibited.
i) Return the test booklet and answer sheet as per instruction.
j) Examinees may ask the in-charge as to when the results shall be
released.
2. Admission Test
2.1 Provisions for New/ Transferee/Returnee Students
a) After step 1 of the Enrollment Procedure, the student may then
proceed at the Guidance Service Office.
b) Look for the test schedule posted at the Guidance Bulletin Board.
c) Submit the accomplished Pre-Enrollment Form with a formal 2x2 ID
picture enclosed to color-coded folder to the Counselor.
d) Examinees may proceed to the Testing Area and observe proper
behavior.
e) Test shall last for40-60 minutes.
f) Results may be released 30-45 minutes after the examination.
g) Results shall be discussed to the examinees.
h) Student may then proceed to the next step as stated on the
Enrollment Procedure.
3. Testing Schedules
3.1 Provisions for New/Transferee/Returnee Students during Enrollment
a) Schedule and time of the admission test shall proceed after the first
step of the enrollment procedure for K-10 students and for SHS

schedule shall be posted/announced electronically via text message


or online posts from the official FB page of MMSC.
3.2 Provisions for Students per Level
a) Psychological Test shall be administered on the following monthly
schedule:
i. Grade 7 Students on August
ii. Grade 8 Students on September
iii. Grade 9 Students on October
iv. Grade 10 Students on November
4. Releasing and Interpretation of Test Results
4.1 Provisions for New/Transferee/Returnee Students
a) Tests may be checked and released after 30-60 minutes
b) The counselor shall interpret the result.
4.2 Provisions for Year Level Test Results
a) Test results shall be consolidated during resume of classes after
semester break.
b) Results of graduating students shall be released before April.

2. RESPONSIVE SERVICES
A. Information Services
It is a method of spreading any kind of information through announcements,
brochures, seminars/symposia/workshops for better adjustment and personal growth
to the whole community.
1. Orientations
This activity can be extended to all faculty, parents, and students.
1.1 Provision for New Students
a) Attend Orientation for New Students scheduled a week before
classes start or within the starting month.
b) Secure a Guidance Services Brochure and Student Handbook at
the Guidance Office and respective advisers.
1.2 Provisions for Newly Hired Faculty/Staff members
a) Attend the Faculty Orientation
b) Secure copies of the following:
1) Student handbook
2) Referral Service/ Guidance Forms
1.3 Provisions for Parents
a) Attend Parents’ General Assembly
b) Parents may visit Guidance Services Office to update their contact
numbersor may request to follow up and monitor their students.
2. Information Drive
These are methods or ways of dissemination.

2.1 Brochures
The counselor shall update and make informative brochures to
distribute to the community. Below are the samples of the Guidance
Brochures:
a) Guidance Services Brochure
b) Referral Service Brochure
c) Welcome Freshies Brochure
2.2 Bulletin Board Display
The counselor shall update the community with posts from the
bulletin board.
2.3 Online Site

3. Enhancement Programs for Students
These are activities prepared by the counselor as requirement for
students to attend.
3.1 Provisions for All Students Enrolled
a) Obtain the following form at the Guidance Services Office
according to your grade level:
1) MMSC GSO-0017: Grade 7 Enhancement Program
2) MMSC GSO-0017A: Grade 8 Enhancement Program
3) MMSC GSO-0017B: Grade 9 Enhancement Program
4) MMSC GSO-0017C: Grade 10 Enhancement Program
5) MMSC GSO-0018A: Grade 11 Enhancement Program
6) MMSC GSO-0018B: Grade 12 Enhancement Program
b) Accomplish the requirements as stated and have it signed by the
Counselor every activity.
c) Submit the form during the final clearance signing before the end
of the semester/school year.
d) The Guidance Counselor shall post monthly activity for the
students.
e) All students shall update themselves with announcements from
the bulletin board and facebook page.
4. Guidance Seminar/Prevention and Wellness Program
These are activities prepared by the counselor according to the needs
assessed from the students. Faculty/Staff may suggest and request for an
activity to be conducted to the students. The office also offers
seminars/workshop to faculty and staff.
4.1 Provision for All Students Enrolled
a) The guidance counselor shall post schedules for guidance
sessions.
b) Interested students may join the session.

4.2 Provisions for Faculty and Staff


1) A faculty/staff may request to conduct a particular topic for a
guidance session to their students.
2) Acquire the MMSC GSO-0020: Request an Activity Form.
3) Fill out the form and submit it to the Guidance Counselor
4) Your request may then be discussed and be updated
especially to the availability of venue.
5. Career Development Plan
A comprehensive program organized and conducted to students in
training them for their chosen field and prepare them in the world of work.
5.1 Career Development Program
It is a program to all Grade 11 and 12 Senior High School and Junior
High School students. It aims to assist the students in choosing a career
based upon personal traits, talents, aptitudes and capacity.
a) Provisions for Career Development Program Coordinator
1) Plan and conduct the whole month program for high school
students.
2) Establish set of topics to empower the students on the
different possible careers and avenue for students to ask
professional of their chosen track/strands/course.
3) Facilitate the whole program of activities.
5.2 Basic Job Entry Skills Seminar and Mock Interview
The activity aims to prepare graduating student in the world of
work most especially on applying for a job.
a) Provisions for Career Development Program Coordinator
1) Plan and conduct the activity for graduating students.
2) Establish set of topics to empower the students on their
chosen careers and possible preparations for employment.
3) Facilitate the whole program of activities.
B. Counseling Service
Counseling is a goal-oriented relationship between a professionally trained,
competent counselor and an individual seeking for help for the purpose of bringing
about a meaningful awareness and understanding of the self and environment,
improving planning and decision making, and formulating new ways of behaving,
feeling, and thinking for problem resolution and/or development growth.
This service is open to MMSC community, for faculty/staff and parents. Just feel
free to come and visit the office and assistance will be given.
1. Ethical Standards
Furthermore, the counselor should adhere to the following ethical
standards:
a) Putting Client’s Needs Before Your Own.

The client’s needs and welfare come first and should be


given priority over the counselor’s needs.
b) The Right of Informed Consent.
The clients have the right to decide whether to enter into
a counseling relationship with the specific counselor and must
be told what to expect. Providing clients with information they
need to make informed choices tends to promote the active
cooperation of clients in their counseling plan.
c) Dimensions of Confidentiality
Counselors have an ethical responsibility to discuss the
nature and purpose of confidentiality with their clients early in
the counselling process. The clients have a right to know that
the counselor may be discussing certain details of the
relationship with a supervisor or a colleague. Moreover, there
are times when confidential information must be divulged.
There is a legal requirement to break confidentiality.
Note the exemptions cited to the ethical principle of
confidentiality:
1) The client is a danger to self or others. The law places
physical safety above considerations of confidentiality
or the right of privacy.
2) The client requests the release of information. Privacy
belongs to the client and may be waived. The
counselor should release information as requested by
the client.
3) A court orders release of information. The
responsibility under the law for the counselor to
maintain confidentiality is overridden when the court
determines that the information is needed to serve the
cause of justice.
4) The counselor is receiving systematic clinical
supervision. The client gives up the right to
confidentiality when it is known that session material
will be used during supervision.
5) Legal and clinical consultation are needed. Again, the
client should be informed of the (ethical) right of the
counselor to obtain other professional opinions about
progress and the name/s of those used as a
consultant/s.
6) Clients raise the issue of their mental health in a legal
proceeding. In a custody suit, for example, parents

introduce their mental condition into the suit,


whereupon they authorized release of the counselor’s
record.
7) Clients are below the age of 18. Parents or guardian
have the legal right to communication between the
minor and the counselor.
8) The counselor has reason to suspect child abuse. All
states now legally require the reporting of suspected
abuse.
d) Personal Relationships with Clients
Relationships with clients that may impair the counselor’s
professional judgements or the client’s response must be
avoided. Counselor must at all times, avoid exploiting clients
for financial gain, social status, research data or other
motives.
2. The Counselee/Client
2.1 Provisions to Student/Counselee/Client
a) Submits willingly to counseling.
b) Fill out the MMSC GSO-003: Appointment Form (please refer to
Appendix D) and give it to the counselor.
c) Read and sign the MMSC GSO-0009: Informed Consent for
Counseling Form.
d) Start of counseling session may proceed.
e) After the counseling session, the counselor and client shall agree
and sign the MMSC GSO-0012: Student’s Satisfactory Academic
Progress Contract
f) Must there be a second or more sessions needed to conduct, the
counselor shall agree with the student of the next date’s schedule.
g) The counselor shall prepare MMSC GSO-0024: Record of
Counseling/Interview for the student to show/give to the faculty
who referred.
h) Follow up will then be conducted to monitor progress.
3. Calling the Student/Client for Counseling
3.1 Provisions for Calling the Student/s
a) Prepare and fill our MMSC GSO-0013: Call Slip to individually call the
student.
b) Obtain schedule of possible time through the adviser before call
slips are served during classes for a quicker and easier way to call
the student’s attention.
c) The subject teacher/adviser may help distribute the call slip to
facilitate faster assistance.

d) As the student received the call slip, he/she then fill out the call slip
receipt as to when he/she may visit the counselor the soonest
available time.
e) If the student is absent, the faculty shall note on the call slip receipt
to document the incident.
f) In cases where the counselee/client does not respond to three (3)
call slips, the counselor may send communication to the
parent/guardian notifying them of the student’s concern.
g) The counselor should avoid calling students during examination
week, or there is a scheduled test or quiz.
C. Referral Service
Teachers, administrators, counselors, parents, and other students may refer
student for counseling.
1. Handling Referrals
1.1 Provisions for Referring Students to a Counselor
a) Identify student’s behaviors indicating the need for help.
b) A student who may need personal counseling would likely manifest
the following behaviors:
1) Performs very poorly in both oral and written exams
2) Shows lack of interest or motivations in his/her studies.
3) Talks aloud and distracts others in class
4) Isolates him/herself from the group
5) Seems to be perpetually tired, anxious, depressed, irritable,
angry, etc.,
6) Manifest poor grooming and hygiene
7) Shows signs of dramatic weight loss or gain, etc.,
8) Incurred three or more consecutive days of absences.
c) If you observe any of these behaviors, inform the student regarding
the behavior patterns you have observed that you deeply concerned
about.
d) The faculty may handle the case as long as it can be managed and no
life is endangered, most especially in the event where the student
chooses not to seek professional help.
e) Suggest that he/she consider availing of the counseling services
provided by his/her counselor. Discuss further the need for him/her
to seek professional help.
f) If the student agrees to the referral, the faculty must inform the
designated counselor through MMSC GSO-0006: Referral Slip. The
faculty has to fill-out the form and give it to the counselor.
g) In case of emergency, the faculty may immediately accompany the
student to the guidance office. Otherwise, you may inform the

student that he/she can meet with the counselor during the regular
working hours. A student has the freedom to refuse a referral.
However, if his/her behavior endangers one well-being and those of
others, it is our utmost responsibility to provide help to such a
student without necessarily getting his/her consent.
h) In case a parent wants to refer the child, they may visit and consult
with the counselor.
1.2 Provisions for Referring Students to a Specialist
These are made for the purpose of referring an individual to a
more qualified person within or outside the school to handle the case.
a) If a Guidance Counselor sees the problem/concern difficult to handle
and is beyond his/her expertise he or she may refer to the roster of
experts to handle the case, provided the client’s decision must be
discussed between the counselor and the client. Referral maybe
made official after the client agrees and fill-out the MMSC GSO-
006B: Referral to a Specialist.
b) The counselor should inform the student of his/her limitations as to
why he/she must refer the student to another counselor/expert.
c) The counselor may refer to another counselor due to certain ethical
reasons re: affiliation with the counselee and/or familial ties.
d) The Guidance Counselor may direct the client to a psychologist/
social worker or another counselor with a higher level of training or
special expertise related to the client’s needs.
e) Those who are manifesting alarming signs/symptoms are referred to
specialists:
1) Speech therapist/pathologist: one who specializes in the
evaluation and treatment of communication/speech disorders
and swallowing disorders.
2) School Clinic: an office wherein students are referred in need of
medicinal assistance or remedial check-ups by the nurse.
3) Social Worker: a person to refer students who are victims of
catastrophes, juvenile delinquents, and suspected child abuse.
4) Psychiatrist and other medical specialists: a person to refer
students who are hooked on drugs, showing signs of mental
challenges, and the like.
2. Referral Feedback:
2.1 Provision for Counselor’s Feedback to Referral
a) After the referral has been made by the faculty, the Counselor
should fill-out the form MMSC GSO-0006A: Referral Feedback Slip to
report the progress of the student.

b) The faculty and counselor may have a regular feedback and


monitoring progress of the student’s academic standing.

D. Follow-up Service
It is a service where counselors call students with behavioral problems,
absences, incomplete school works, failed subjects, and other problems pertaining
studies for academic consultation. It also includes academic monitoring, and counselor-
parent/guardian and counselor-teacher consultation to facilitate concern affecting
school performance and self-management.

Furthermore, to check how the students are doing and assess if further
assistance is needed.
1) Provision for Academic Follow-up
a) Advisers shall submit a list of students to be followed-up due to
failed/incomplete/dropped subjects.
b) Advisers may fill out the form MMSC GSO-0022: List of Students for
Academic Follow-up.
c) The list shall be submitted during the end of each grading period, and
the first month of every semester for SHS. Thus, students shall be
followed-up are those who are not doing well in the previous grading
period/semester.
d) The counselor shall call and follow-up the student listed through MMSC
GSO-0022: Academic Follow-up Form and submit a report to the
Principal.
2) Provision for Academic Performance Monitoring
a) If the student has history in having poor attendance or academic
performance and a scholar, the counselor may perform and facilitate
MMSC GSO-0005: Academic Performance Monitoring Form.
b) Teachers of the student may then fill-out necessary details as to when
the student was absent, as well as his/her academic performance.
c) If two or more subjects are failing, the counselor may call the student for
consultation/counseling and may opt to call the parents as well.
3) Provision for Student Monitoring
a) Student shall be monitored yearly by the counselor by means of routine
interview
4) Provisions for Personal Follow-up by the Counselor
a) After the student has been counseled, it is the duty of the counselor to
follow-up for any development or further assistance needed.

E. Consultation & Collaboration


It is a service where counselor and school administrators/faculty/students/
Parents help each other to reach a desired goal most especially towards a holistic
development of students.
1. Provision for Calling Parents to a Case Conference
a) The counselor must explain the reason to the counselee the need to
have a case conference for permission.
b) A Request for Case Conference/ Conference Letter (MMSC GSO-0018)
shall be accomplished by the counselor and may be sent to the
parent/guardian/faculty. A text message shall also be sent regarding the
appointment.
c) The request shall include the time and venue and confirmation slip/text
confirmation
d) The parent/guardian/faculty together with the counselor shall discuss
the situation of the counselee.
2. Provisions for Faculty/Staff-Counselor Consultation
a) A faculty/staff may have a consultation with the counselor to discuss
about a student’s unusual behavior observed before a formal referral
shall be made.
b) The counselor shall ask questions about other observed behaviors and
may call for all the faculty involved to assess the student’s behavior in
other subjects.
3. Provision for Counselor-Administration/Academic Council Collaboration
a) The Guidance Counselor shall present the consolidated reports from the
students the office has gathered.
b) Results may be discussed together with the administration or academic
community to address the student’s need with hopes to enhance
services.

III. SYSTEM SUPPORT


A. Peer Outreach Program
1. Provision for Peer Outreach Program Coordinator
The coordinator is appointed by the Principal as part of their other
function aside from being a Counselor.
1.1 Functions
a) Plans activities for the program annually with the presence of the
officers.
b) Conducts interview for the newly registered members of the
organization.
c) Acts as consultant of the officers as well as the members.

d) Monitors the performance of the officers as well as the over-all


performance of the organization
e) Plots activities that benefit the school community and enhance
members’ leadership and facilitating skills.
f) Makes and evaluates Peer Outreach Program annually.
g) Organizes training workshops and teambuilding activities.
h) Coordinates with the Guidance personnel for possible collaborations.
2. Provision for Peer Outreach Program
a) To help disseminate and carry out the various services while realizing the
felt need to be of service to a greater number of students.
b) To help correct the common misconceptions regarding guidance and
inculcate a more positive and appropriate attitude in the hearts and
minds of students and faculty.
c) To improve the student’s perception towards guidance counselors as
authority figures placed on the pedestal, hence threatening.
d) To facilitate the maximization of students’ awareness of the existence of
a guidance office whose main concern is the holistic growth and
development of an individual as a person with worth and dignity.
e) Peers have more time to be with one another thus, have more chances of
interaction and opportunities to observe behaviors than the guidance
counselor.
f) The peer facilitator acts as the “referral agent” or the bridge between the
guidance office and reluctant students, between students themselves
and even between faculty and students.

B. Career Development/Placement Service


1. Provision for Graduating Students
a) All graduating students shall download the form MMSC GSO-0007: Job
Placement form and enter all important details.
b) The office together with the Alumni Office shall create a database for
students who finished for a specific school year.
c) Information may be used to contact graduates and inform them of any
job hiring activities.
2. Provision for Graduates/Alumni
a) Announcement of job hiring activities will be posted at the MMSC
Guidance Services Office facebook page and the Guidance Bulletin Board
b) MMSC community shall help distribute the Tracer Form (MMSC GSO-
0025) to all the graduates to be able to facilitate or recognize them in any
means the school could give.
c) Every year, the school shall trace the school year 3 years backwards.

C. Research and Evaluation


To obtain valid and reliable data needed for area of improvement and
development, the Guidance Counselor conducts evaluation of its programs and
services.
1. Provisions for Guidance Program Evaluative Report
a) Guidance Counselor shall evaluate the Annual Guidance Program
at the end of the school year.
b) The report shall be submitted to the Principal, copy furnished to
the Director.
2. Provisions for Accomplishment Reports
2.1 Annual Accomplishment Reports
a) The counselor shall prepare and submit the Annual
Accomplishment Reports every 27th of April before the start
of classes.
b) The report will cover the months from June-March
accomplishments.
c) It shall be submitted to the Principal and Director.
2.2 Monthly Accomplishment Reports
a) The counselor shall prepare and submit the Monthly
Accomplishment report every 27th of Month.
b) It shall be submitted to the Principal and the Director.
3. Provisions for Program/Service Evaluative Reports
3.1 Seminar/Workshop
a) Every after seminar/workshop is conducted, the counselor
must give and Evaluation Form (MMSC GSO-0015) to the
participants for an objective opinion about the seminar
administered.
b) The data will be summarize and reported for each seminar.
c) Consolidated report shall be made after the school year ends
and be submitted to the Principal, copy furnished to the
Director.
3.2 Guidance Office Services
a) Every client shall answer the Feedback Form (MMSC GSO-
0015) for the Guidance Services Office to improve and
provide better services to the clients.
b) A consolidated report shall be made every after the end of
the semester and be submitted to the Principal, copy
furnished to the Director.
D. Community Extension Program

To create opportunities to contribute and educate the community.


1. Provision for Community Extension Service.
a) The Guidance Counselor may accept any invitation as Resource Speaker
most especially on Strengthening Relationships/Parenting/Mental
Health and other related areas or as Facilitator to conduct teambuilding
activities.
b) The person/department/agency who requests of the service with a
formal invitation letter shall fill-out the Request for Guidance Extension
Services Form (MMSC GSO-0024) and have it approved by the Principal.
c) Discussion of the schedule and venue shall be done with the requesting
party after the approval.
