Description

SUBCONTRACTOR to provide all documents and drawings for review / approval by PERTAMINA until the
"Issue For Construction" document during the project implementation
SUBCONTRACTOR must provide all temporary storage needs, construction facilities, buildings, health or care
facilities, camps and lodging facilities and other construction infrastructure
SUBCONTRACTOR must provide all construction equipment and tools needed / required in order to carry out
all construction work so that it can be achieved
SUBCONTRACTOR must carry out construction in the refinery area in accordance with the approved drawings
and specifications and meet the safety aspects and applicable local regulations

SUBCONTRACTOR (together with SCHNEIDER ELECTRIC) must carry out all testing and certification activities
together with PERTAMINA up to the date of the Commercial Completion Certificate, and must perform all
obligations under this contract to fulfill the warranty
SUBCONTRACTOR must provide office space, furniture, equipment and all office equipment, including
telephone, telex, facsimile, communication, on site and local areas to facilitate coordination and any site
meetings
MAIN SCOPE:
1. Modification internal wiring existing BO5 w/q 11 Pcs.
2. Provide materials and accessories needed for modification existing BO5, included and not limited to cable,
cable duct, ferrule, etc
3. Provide new mounting plate, DIN Rails and accessories for existing BO5
4. Dismantling existing BO5's mounting plate
5. Dismantling existing IO Module and existing Baseplate
6. Install new mounting plate with accessories
7. Install new baseplate and IO modules (include wiring) on exisitng BO5
8. Provide 1 (one) new BO5 cabinet (refer to Foxboro Global Enclosure G06, Stainless steel) wiring and
accessories, include cabinet support included but not limited to canopy, accessories, etc.
9. Install new BO5 and accessories on site
10. Install, laying, Pulling, splicing, OTDR test for FO Cable for FCP and FCM
11. Dismantling Existing ATS, install new ATS w/q 13 Pcs included but not limited to cable and accessories.
12. Laying and pulling Power Cable from cabinet rack room to console desk.

HSE & Fire Protection Work in Implementation of Work
a. SUBCONTRACTOR must compile and prepare all HSE documents related to the scope of the project
implementation.
b. SUBCONTRACTOR must provide all safety equipment related to the scope of the implementation of this
project.
c. SUBCONTRACTOR must provide a Safety Officer and Safety Man with competencies in accordance with the
requirements of PERTAMINA (Persero) HSE and be adjusted to each Refinery Unit in sufficient quantities.
d. SUBCONTRACTOR must comply with RU-IV, RU-V and RU-VI environmental regulations as well as the
applicable external environment and ensure that there is no environmental damage or waste disposal that
violates the rules at each stage of the construction work it carries out.

HSE Procedure
SUBCONTRACTOR must prepare HSE Plan and Job Safety Analysis (JSA) documents, HSE procedures for all
potential hazards that may arise, how to prevent and deal with this project work, all HSE document must get
SCHNEIDER ELECTRIC and PERTAMINA approval
Safety and Fire Protection Facilities
a. Safety equipment standards must refer to the regulations in force
PERTAMINA
b. Provide Personal Protective Equipment (PPE) equipment for workers in accordance with their respective
work areas.
c. SUBCONTRACTOR make signs for evacuation routes and coordinate with PERTAMINA
d. Hold safety signs (safety signs) in the form of written suggestions, hazards and warnings, safety lines, traffic
cones in sufficient quantities.
e. Providing Light Fire Extinguisher (APAR) in the work area that has the potential to cause a fire hazard in
sufficient quantities at each work location (Refinery Unit)
f. Provides Fire protection and fire blanketing equipment for hot work types.
g. For work at SUBCONTRACTOR heights, workers must complete their work with a full body double hook
harness
Safety Superintendent
a. Manage licensing before the work takes place.
b. SUBCONTRACTOR are required to provide a Safety Officer and Safety Man in accordance with the
competencies and qualifications in accordance with the requirements of PERTAMINA
c. Safety Supervisors must supervise and be responsible for work safety and equipment during the
construction period.
d. Safety Supervisors must always coordinate with HSE PERTAMINA
e. SUBCONTRACTOR (together with SCHNEIDER ELECTRIC) must carry out a Safety Talk Meeting for all
workers at the start of the project.
f. SUBCONTRACTORS must carry out the Tool Box Meeting every morning, before starting work.
g. Subcontractors must take care of the safety tags to PERTAMINA HSE on all equipment used in this project
in accordance with applicable regulations, before the tool is used.
h. Make a report HSE activity per week during the implementation of construction work.
Administration, Licensing and Mobilization / Demobilization Work
a. SUBCONTRACTOR prepare all administrative work and take care of all necessary permits to PERTAMINA
REFINERY
b. Together with the SCHNEIDER ELECTRIC team, they prepare a work plan and supply of manpower and
equipment in accordance with a work schedule in sufficient quantities.
c. SUBCONTRACTOR must carry out mobilization and demobilization of labor, materials and all equipment at
the cost of SUBCONTRACTOR.
d. SUBCONTRACTOR must insure (BPJS) for all workers involved in the work of this project to the insurance
company include Contruction & Service insurance (JAKON)
e. SUBCONTRACTOR must insure the entire scope of work with Construction All Risk, including Third Party
Liability, to insurance companies approved by PERTAMINA with a coverage limit value of the contract value.
f. Provide work equipment and material transportation equipment as well as SUBCONTRACTOR workers from
and to SPM regardless of the RUs in accordance with sufficient quantities.

General Licensing and Administration


a. All costs arising from administrative preparation and mobilization work are the responsibility of the
SUBCONTRACTOR.
b. SUBCONTRACTOR prepare all administrative work and take care of all necessary permits including
materials, labor and work equipment to PERTAMINA, as well as to related agencies
c. For SUBCONTRACTOR who come from outside the working area of PERTAMINA, must have a
Representative Office in the region which can be contacted at any time. SUBCONTRACTOR notifies in writing
to PERTAMINA , Personnel responsible, Full Address, Telephone Number
d. Carry out preparatory work in the field which includes reviewing work sites, detailed field surveys and
preparation of work equipment
Warehouse Work
a. SUBCONTRACTOR are required to prepare a minimum of 1 (one) Warehouse unit each RU for the needs of
SUBCONTRACTOR, according to the instructions of PERTAMINA supervisors
b. After the entire project is complete, all warehouse facilities are dismantled and disbursed at the expense
of the SUBCONTRACTOR.
c. Warehouse materials and equipment belong to SUBCONTRACTOR.
d. SUBCONTRACTOR are required to tidy up the former warehouse location
Work and Communication Equipment
a. SUBCONTRACTOR must prepare office and construction work equipment including:
1. Office stationery supplies.
2. Construction work equipment and tools.
3. Other supporting equipment needed.
b. SUBCONTRACTOR are required to provide / lease communication equipment in the form of radios that
meet the standard Class A Div 1 of at least 2 (two) units in RU and can be connected with related function
channels in PERTAMINA for the benefit of work implementation and coordination
Mobilization and Demobilization
a. Vehicles used for mobilization and demobilization must meet the applicable requirements in PERTAMINA
the maximum age of the vehicle for the last 5 (five) years
b. Transport equipment used must comply with traffic regulations, Government Regulatory (PERDA)
applicable in the area traversed, including the capacity and type of transportation that is permitted to take
local public roads.
c. If PERTAMINA assesses that the equipment used is insufficient and is expected to disrupt the
implementation schedule, PERTAMINA has the right to order to bring in additional necessary equipment at
the expense of a SUBCONTRACTOR.
d. All work equipment, waste materials and labor that are no longer needed on site must be removed from
the project site after obtaining written permission from PERTAMINA , except the tools that are still needed
during the Maintenance Period.
e. All costs incurred and risks during the implementation of mobilization and demobilization in this work are
the responsibility of the SUBCONTRACTOR
a. SUBCONTRACTOR must create a project organizational structure complete with their respective personnel
and scope of work.
b. SUBCONTRACTOR must provide Site Manager personnel

d. SUBCONTRACTOR must submit project team, personnel CV withs its certificates and project schedule to
show their capability
The nature of this work is LUMP SUM
SITE SURVEY
Shall be vendor scope for all site survey cost
PROCUREMENT AND FABRICATION OF MATERIAL / EQUIPMENT
a. Procurement / supply of all materials and equipment needed in this project work including inspection,
testing and shipment.
b. Procurement of spare & consumable parts for commissioning
c. In the procurement of all imported and local materials, the contractor must refer to the brands listed in
the "Approval Manufacture List" issued by PERTAMINA's latest revision. For this matter, in the material /
equipment offer, the contractor must mention the brand / brand of the material / equipment concerned,
except for the category of material / equipment not listed in the "Recommended Brand List", then the brand
/ brand of the material / equipment category will purchased must obtain PERTAMINA approval. For general
material (not brand), the contractor submits the specifications of the material to be purchased in a letter of
approval to PERTAMINA.
d. Delivery of material / equipment which is critical must be proven by a certificate of Country of Origin and
Contry of Manufacture
e. All materials and equipment imported by Subcontractors must be in a new condition, and in accordance
with the required specifications.
f. The material in question must be accompanied by specifications documents, certifications and reports of
test results from the manufacturer. If deemed necessary, PERTAMINA can request a retest carried out by an
independent legal entity at the expense of the SUB-CONTRACTOR.
g. SUBCONTRACTORS must procure / bring all material to the site in sufficient quantities, believed to be in
new conditions and in accordance with the specifications requested to carry out this work.
a. The SUBCONTRACTOR must sketch the test and be approved by
SCHNEIDER ELECTRIC and PERTAMINA.
b. All results of SUBCONTRACTOR work must be tested in accordance with applicable standards and
regulations as well as technical specifications.
c. SUBCONTRACTOR must appoint a coordinator and quality control (Quality Control) who truly understand in
achieving good work quality. SUBCONTRACTOR must submit QC CV and certificate for SCHNEIDER ELECTRIC
review and approval.
d. A quality controller has the authority to stop work that is not in accordance with procedures. The duties of
the quality supervisor include the following:
1. Prepare a detailed plan for inspection / testing procedures.
2. Checking / overseeing the steps of implementing the test.
3. Prepare test equipment that has been calibrated.
4. Prepare personnel qualification records and welding work.
5. Notify / contact PERTAMINA Inspection during the test to witness the test results.
e. All results of SUBCONTRACTOR work that do not meet the provisions and standards stipulated by
PERTAMINA must be improved so that they can meet the provisions and standards.
f. All costs incurred for carrying out the inspection, testing and repair mentioned above are the responsibility
of the SUBCONTRACTOR
h. All costs incurred for the procurement of materials and equipment mentioned above, including import
fees, custom clearance, import duties, licensing and others become the burden and responsibility of
SUBCONTRACTOR.
i. Material / equipment specifications that will be used must obtain PERTAMINA's approval before being held.
Material / equipment that is held without the approval of PERTAMINA may be refused and all costs incurred
will be the responsibility and burden of the SUBCONTRACTOR.
j. All materials /PERIOD
MAINTENANCE equipment that will be used must be equipped with a Material Certificate and will be
a. The maintenance period is 180 (serratus eighty) calendar days counted from the date of the First
Handover.
b. During the maintenance period, if there is damage or imperfections due to imperfections of the system
and its construction, the SUBCONTRACTOR is obliged to repair the costs of the SUBCONTRACTOR (material
and wages).
c. This maintenance period can be more than stated in the contract if the repair of damage that occurred
during the maintenance period has not been completed and in this case the SUBCONTRACTOR is still obliged
to carry out repairs at the expense of the SUBCONTRACTOR until all damage can be overcome and good trial
/ operation has been carried out individually or systemically and proven to be successful with a minimum of
continuous operation for 15 calendar days after repairs and is well received by PERTAMINA.
d. After completing the maintenance period, the Second Handover will be held
WARRANTY
a. The warranty period is for 12 (twelve) months starting from the date the signing of the Official Report on
Handover Phase I is signed.
b. SUBCONTRACTOR guarantee, that the work carried out truly meets the requirements of this work and its
specifications and is always in prime quality, free from imperfections in construction, work skills, and Design
that must be provided by SUBCONTRACTOR.
c. SUBCONTRACTOR are required to submit Guarantee Certificates from Manufacture / Vendors to Schneider
Electric for a minimum of 12 (twelve) months after the first submission.
d. Within the 12 (twelve) month Warranty Period SUBCONTRACTOR cooperates with the agent / vendor who
is obliged to repair / replace the cost of the SUBCONTRACTOR, equipment that turns out to be imperfect /
not functioning as planned according to the smallest idle time of the equipment.
e. Within the 12 (twelve) month Warranty Period SUBCONTRACTOR cooperates with agents / vendors who
are obliged to repair / replace the cost of SUBCONTRACTOR, equipment that turns out to be defect quality so
that it has not functioned as planned according to the specifications requested in the scope of this work

CLEANING THE WORK AREA AND IMPROVING EXISTING FACILITIES.


a. Clean the work area to the specified place.
b. Transfer of directors to the warehouse and warehouse
c. Remove all remaining project material from PERTAMINA Refinery.
d. Carry out repairs / replacements of existing facilities / equipment damaged due to this work and return
them according to the initial conditions at the expense of the SUBCONTRACTOR.
COMMERCIAL TERM
a. Liquidated Damages 1% per day or maximum 10% from Purchase Order Value
b. Payment Term
Term 1: Pembayaran tahap pertama akan dibayarkan sebesar 90% dari nilai kontrak setelah semua peralatan
selesai terpasang dan berhasil operasi dengan baik serta diterima oleh Pertamina
Term 2: Pembayaran tahap kedua akan dibayarkan sebesar 10% dari nilai kontrak setelah masa pemeliharaan
dan tidak ada tanggungan/sisa pekerjaan yang harus diselesaikan

e. Warranty
- Pihak Subkontraktor harus menjamin bahwa system yang terpasang harus dapat beroperasi
sesuai dengan spesifikasi dan segala penyimpangan yang terjadi adalah menjadi tanggung jawab
Subkontraktor.
- Subkontraktor harus menyerahkan performance guarantee (certificate guarantee) dari manufacture /
vendor kepada Pertamina untuk peralatan dominan (peralatan instrumentasi) sesuai standard
yang berlaku minimal 12 (dua belas) bulan.
- Dalam Masa Garansi selama 12 (dua belas) bulan sejak ditandatanganinya penyelesaian
pekerjaan (BPAST-1), Kontraktor berkewajiban untuk melaksanakan maintenance support.
