10COMES NOW Xxxxxx Xxxxxxxx (hereinafter “I,” “me,” “my,” “mine”), being the undersigned affiant, of
11sound mind and age of majority, having first-hand knowledge and being competent to testify, and after
12being duly sworn, states under oath, to wit:

14 1. The arresting officer admitted no probable cause or reasonable suspicion to believe that I was
15 operating a motor vehicle in commerce during the events upon which this action is based.
16 2. I was merely traveling by right in my family automobile when arrested during the events upon
17 which this action is based.
18 3. I have not been served by the agency with any notice of accusation or hearing related to the facts
19 upon which this action is based.
20 4. I have been denied my right to request a hearing.
21 5. I have been denied my right to object to the accusation.
22 6. I have been denied my right to object to the form of the accusation on the ground that it is so
23 indefinite or uncertain that the respondent cannot identify the transaction or prepare a defense.
24 7. I have been denied my right to a hearing on the merits of the accusation which was never served
25 upon me in the first place.
